As a member of the hospice interdisciplinary team, the Hospice Chaplain provides spiritual and religious support to patients and families consistent with their assessed needs, requests and belief system at end-of-life. The chaplain also serves as a spiritual resource within the agency providing support and guidance for staff.
Essential Functions
- Participates in the interdisciplinary team conferences and assists in developing and reviewing care plans
- Completes an initial spiritual assessment as required by Medicare Conditions of Participation guidelines.
- Based on assessment and patient/family wishes, provide spiritual support and interventions in nonjudgmental manner.
- Participates in patient care conferences, bereavement meetings, training programs and other events as appropriate and needed.
- Serve as a liaison with notification and involvement of patient/family’s own spiritual community at patient/family’s request.
- Understands family dynamics and works effectively within various types of family systems.
- Communicate patient/family reports concisely and effectively to IDT.
- Assists with funeral and memorial services when requested.
- Documents all contacts per procedure including visits, telephone calls, assessments, interventions, responses to interventions, and communications.
Position Type/Expected Hours of Work & Travel
This is a full-time position, and days and hours of work are Monday through Friday, 8:00 a.m. through 4:30 p.m.
- Master of Divinity degree or equivalent from an accredited school/seminary required with completion of one unit of Clinical Pastoral Education.
- Endorsed standing as a minister, priest, rabbi, or member of a religious order preferred.
- Experience in a health care setting or working with end-of-life care preferred
