Elizabeth Layton Center is looking for an Integrated Care Case Manager. This position is full-time Monday - Friday. EOE
ELC is a private, not-for-profit community mental health center serving the citizens of Franklin and Miami counties in Kansas. We have 4 locations and serve over 4,400 clients yearly.

SUMMARY:
The Integrated Care Case Manager will work with ELC consumers with mental health and physical health conditions, as well as substance use disorders. Focus of care will be on coordinating behavioral and physical health needs with other service providers, including case managers, physicians, therapists and medication providers, as well as providers from other agencies. This position reports directly to the Quality Assurance Director, or designee.
MAJOR JOB RESPONSIBILITIES:
  • Provide community based and/or telehealth services to consumers identified as needing assistance with specific behavioral health and physical health symptoms. Some of these activities include such things as symptom management, care coordination and daily living skill development.
  • Work with consumers to determine services/activities needed to achieve successful outcomes (e.g. recommended follow-up, improved symptoms, and improvement in medical and psychiatric conditions)
  • Assist clients in identifying available resources in the community, advocating for access to care, and identifying natural supports.
  • Engage consumers in a variety of ways to increase understanding of health condition/health literacy and motivation to engage in self-management through both individual and group contact.
  • Encourage consumers to monitor and manage their health, emphasizing self-direction and skills development.
  • Provide health education and coaching to consumers, families and support persons and ELC staff.
  • Collect, track, enter and follow up on data related to incentive-based MCO metrics, including, but not exclusive to discharge appointments, diabetes and other health screenings, ER utilization, and medication management.
  • Complete all paperwork associated with the financial, clinical, legal or regulatory management of the case or ELC’s operations in a timely and accurate manner.

EDUCATION and/or EXPERIENCE:
  • Bachelor’s degree (social work, psychology, human services, etc.) or equivalent experience
  • Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards and the OneCare Kansas program manual.
  • Familiarity with mental health theory and practice
  • Familiarity with physical health theory and practice preferred
KNOWLEDGE, ABILITIES, AND SKILLS:
  • An energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
  • Knowledge of the medical and non-medical service delivery system within and outside of Franklin and/or Miami County.
  • Good team-building relationship skills, time management, and deadline management.
  • Professional and effective written and oral communication and management skills.
  • Ability to work with complex spreadsheets and population health software.
  • Strong interpersonal skills in working with diverse publics.
  • Strong organizational skills.
  • Reliable transportation; and a valid, unrestricted driver’s license.
  • An ability to work independently and in cooperation with others.
  • Ability to exercise sound independent judgment.
  • Ability to safely perform major job responsibilities with or without reasonable accommodations.
  • Some travel required.
