SUMMARY:
Provides community-based and mobile service delivery with the purpose/intent to offer specific, measurable, and individualized services to each person served. TBS services are focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work, and family and integration and contributions within the community.
MINIMUM QUALIFICATIONS:
- A High School Diploma or G.E.D (for QBHS)
- At least 6 months of experience providing case management/qualified mental or behavioral health specialist service
Preferred Qualifications
- Associate's or Bachelor’s degree in Social Work, Human Services, or related field preferred; experience providing TBS/Case Management services (for QMHS)
CORE COMPETENCIES:
- Providing comprehensive care coordination,
- Assisting adults, and their families with skill building
- Crisis management
- Providing linkages to community resources such as housing, transportation, vocational, medical, and financial services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Ongoing assessment of needs.
- Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian.
- Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian.
- Coordination of the ISP, including.
- Services identified in the ISP.
- Assistance with accessing natural support systems in the community; and
- Linkages to formal community service/systems.
- Symptom monitoring.
- Coordination and/or assistance in crisis management and stabilization as needed.
- Advocacy and outreach.
- As appropriate to the care provided to individuals, and when appropriate, to the family, education, and training specific to the individual's assessed needs, abilities, and readiness to learn.
- Mental health interventions that address symptoms, behaviors, thought processes, etc., assist an individual in eliminating barriers to seeking or maintaining education and employment.
- Activities that increase the individual's capacity to positively impact his/her own environment.
ESSENTIAL JOB REQUIREMENTS:
The following are the requirements of this job and must be met and maintained at all times during employment in this job:
- No felony convictions in the last two years
- Ability to pass a drug and alcohol screen per the drug-free workplace policy
- Ability to pass criminal background security screenings as required by regulations/laws related to:
a. Performing the essential duties of this job
b. Working in and accessing partner institutions such as criminal justice facilities, schools, residential facilities, etc.
c. Working in and accessing an MTHSS institution such as a residential center, senior center, etc.
- Possession of all valid professional licenses required by regulations/laws and accreditation related to performing the essential duties of the job.
- Possession of a valid Ohio driver’slicense
- Possession of an automobile drivers insurance policy that meets state minimum requirements
- Possession of an automobile with valid plates which meet state requirements
- Ability to perform the following for extended periods, periodically and frequently:
a. Lift a minimum of 10 lbs.
b. Stoop
c. Kneel
d. Bend
e. Feel
f. Walk; and
g. Stand.
- Remain free from being listed by a State, County, City, or federal organization as excluded, debarred, suspended, or otherwise ineligible to participate in government-funded programs including but not limited to Medicare, Medicaid, Childcare Vouchers, Senior Services, Child Welfare or other.
- Remain free from being listed on the Department of Health and Human Services Office or the Inspector General Cumulative Sanctions Report.
- Remain free from being listed on the General Services Administration List of Parties Excluded from the Federal Procurement and Non-Procurement Programs.
Job Type: Contract
Salary: $20.00 - $30.00 per hour
