• Position: LPN or Licensed Practice Nurse
• Location: Bronx, NY
• Schedule: Flexible, Morning, Mid shift and Night shifts available
• Salary : $40/hr

Duties:

• Establish for the adolescents in care a medical home that will address current and future health care needs.
• Administer medication and monitor the youth's response to prescribed medication Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care in the department/program to create a non-violent and trauma-sensitive culture.
• Work with family and youth to develop the most appropriate permanency plan with emphasis on lasting family connections, which can be, improved family relationships, adoption, mentor , etcWork with the Adolescent Services team for the comprehensive service planning for each youth, including education, employment and other PYA skills.
• Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care, when applicable, in the department/program to create a non-violent and trauma-sensitive culture.
• Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
• Implement and integrate the principles and procedures of Solution Based Casework: assessment, family and individual level outcomes, action plans and documentation and celebration.

Qualification

• Graduate of an accredited practical nursing program.
