Memorial Medical Group is looking for an energetic, committed, mission-driven Behavioral Health Care Specialist. Memorial Medical Group embraces an integrated model of care that utilizes a team approach that include a team of clinicians to provide primary and behavioral support. Candidate will be responsible for primary care referrals for behavioral health counseling within each individual practice. Due to the growth of the behavioral health needs we welcome candidates in a position of autonomy and expertise to treat a variety of mental health diagnoses in a private setting.

We focus on patient outcomes and successes, so there is no set productivity that needs obtained. We have had success with patients being more open to seek counseling within the walls of their primary care office. So now is the time to expand these services.

Properly documents and codes visits as appropriate; Facilitates outbound referrals from start to finish; Identifies daily patient assignment through practice referrals; Conduct’s age appropriate assessments pertaining to the psychosocial, economic, and home environment of high- risk medical patients; Provides further age appropriate psychosocial assessment, identifying functional, economic, cultural, and safety needs while assisting in establishing a treatment/care plan when requested by the patient, physician, or nursing services through use of health assessment triggers, (E.g. domestic violence, elder and child abuse/ neglect or substance abuse, suicidal, homicidal behavior, extended care placements, code status discussions etc); Communicates patient needs with patient and /or family to gain cooperation and involvement with treatment; Provides education, recommendation, and/or counseling pertaining to diagnosis and community resources; Advocates for patient rights; Provides education to patients, staff and public on self- determination issues; Records patient evaluation and progress information in the medical record; Actively communicates pertinent patient information to other members of the health care team; communicates with physicians; interacts with ancillary services; coordinates, collaborates, and functions as a team member with other providers of care to deliver effective continuous patient care; Documents accurately (painting a complete picture providing directions to other health care providers); Refrains from using unapproved abbreviations in written or computer documentation; Documentation is completed on a daily basis; Understands the importance of accurate completion of documentation and standards for accrediting body requirements and how it impacts payment; Maintains accurate records. (Aprima, e-MD’s, Time cards, etc.) Travels to and gains access to various practice locations; Assesses health care needs (e.g. psychosocial and educational needs); Instructs patient and or family in conducting prescribed treatments at home when indicated; Makes recommendations to the patient and family /caregiver; Involves family members and significant other in the process; provides kindness and consideration in meeting the emotional needs for patients; confers with physicians; interacts with staff; Develops and applies computer skills (Microsoft Office, Paragon, Aprima, e-MD’s) to enter and retrieve information; Keeps current on new legislative information, new procedures and safety information to provide a safe patient care environment; Actively participates in community outreach programs providing supportive and networking services. Attend and actively participate in all required training and meeting activities, which includes travel between offices. Maintains complete confidentiality of all patient medical records; Maintain open and effective communication with providers and other employees to ensure quality patient care.

Requirements
Completion of an accredited school of social work with current active registration in the State of Ohio with good standing, 3-5 years of experience preferred. Masters in Social Work, Licensed as an Independent Social worker in the State of Ohio; completes 30 contact hours of approved CE during each two year renewal period consistent with requirements of the Ohio Board of Social Work. Maintains continuous certification in American Heart Association’s BLS.
