The Longitudinal Care Transitions Program Coordinator assists with the planning, implementation and coordination of all activities within our newly established program to track and improve transitions across our care continuum for for children with medical complexity. This will be a dynamic position, serving as a liaison for leadership throughout our organization and facilitating engagement with families of the children and young adults we serve. Supports Hospital's mission, vision and values and complies with regulatory guidelines & agency standards.
JOB DUTIES / RESPONSIBILITIES
ESSENTIAL DUTIES:
- Coordinates and monitors daily activities within the department.
- Responsible for daily data entry and tracking for all departmental metrics.
- Creates and disseminates departmental reports with supporting data display (charts & graphs).
- Acts as the liaison between department and each service line and external partners to ensure timely communication and data sharing.
- Schedules interdepartmental meetings and family engagement activities.
- Actively participates in project development and management.
- Assists in conducting market research and documents findings.
- Assists with the preparation of in-service material/staff resources and family resource materials.
- Coordinates programs that support the Longitudinal Care strategy, including coordination of research projects.
- Assists with the development of policies and procedures to support efficient and effective departmental activities.
- Performs various clerical tasks as necessary, including typing, copying, and distribution.
Formal education required:
Associates Degree required; Bachelors Degree preferred
Previous experience required:
Minimum of 1 year health care experience, care coordination experience preferred.
Experience with and understanding of New York State health care regulations and policies.
Familiarity with Patient Centered Care and Care Coordination desirable.

Pay: $45,000.00 - $50,000.00 per year
