Key Responsibilities:

• Collaborate with interdisciplinary teams, including psychiatrists, psychologists, nurses, and other healthcare professionals, to coordinate and implement patient care plans.
• Assist in developing discharge plans for patients, including coordinating community resources, providing referrals for appropriate aftercare services, and assisting with transitions to community-based settings.
  
• Provide individual and group counseling to patients to help them cope with mental health challenges, develop coping skills, and improve their emotional well-being..

Qualifications :

• Licensed Clinical Social Worker (Required)
• Master’s Degree in Social Work with 1+ years of healthcare/medical social work experience
• Proficiency in Microsoft Office suite
