Mountain View Hospital is looking for a Medical Records Clerk to join our team!


JOB SUMMARY:

Receives and scans all incoming medical records into ARIA/EMR. Ensures that all scanned documents and electronically filed into correct note type with description, verifying name and DOB for accuracy. Ensures that all transcribed reports are scanned and filed in a timely manner in the charts. Logging, tracking and processing all incoming medical records requests, following HIPAA guidelines. Compiling all necessary information for external and internal audit requests. Competing records request received daily from other physicians office for more immediate scheduled appointments.

DUTIES AND RESPONSIBILITIES:

The H.I.M. Clerk:

  • Receive and scan all incoming medical records into ARIA/EMR.
  • Log, track and process all incoming medical records requests, following HIPAA guidelines. Verify there is a current signed Records Release form on file for the patient.
  • Log request for tracking. Only results generated from physicians at this office/practice can be sent. All patient files can be shared if requested by a patient or are being sent to another facility for continuation of care. Verify there is a current signed Records Release form on file for the patient.
  • Review patient’s file for documents requested and applicable dates. If applicable, calculate and submit the charge for the records being sent. Create invoice and billing letters to submit when the records are being requested.
  • Complete external and internal audit requests, i.e. checking the requested files and copying the appropriate information.
  • Complete records requests daily from other physicians’ offices, by priority scheduled appointments.
  • Assist MVH records departments with records requests.
  • Completes annual education requirements.
  • Reports to work on time and as scheduled, completes work within designated time.
  • Responsible for knowing hospital policies and being familiar with hospital services that are made.
  • Demonstrates an ability to be flexible, organized and function well in stressful situations.
  • Treats patients and their families with respect and dignity; ensures confidentiality of patients’ records.
  • Maintains a good working relationship both within the department and with other departments.
  • Participates in educational programs, in-service meetings and completes annual educational requirements.
  • Attends all other meetings as required.
  • Maintains patient confidentiality at all times.
  • Accepts and completes other duties as assigned or requested.

Education/Certification: High school diploma or general education degree (GED).


Experience:
One to three months related experience and/or training; or equivalent combination of education and experience preferred.


Equipment/Technology:
Ability to use hospital information system, and operate basic computer functions and software.


Language/Communication:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.


Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, percent and to draw and interpret bar graphs.


Mental Capabilities:
Ability to apply common sense to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
