Job description
POSITION SUMMARY
The Population Health Specialist is an essential member of the health care team that supports the activities of the Quality and Population Health Departments through various clerical duties. Excellent time management and an ability to work as part of a team are essential skills for this role. The Population Health Specialist reports to the Population Health Program Manager. Assists with Quality & Value-Based Programs departmental operations and initiatives. Maintains working knowledge of quality programs the Health Center participates in, the Health Information Technology systems used, and workflows to support HEDIS and quality improvement activities. Supports compliance with quality program requirements as outlined by Centers for Medicare & Medicaid Services, New York State Department of Health, and National Committee for Quality Assurance, among others. Performs outreach and education projects related to HEDIS and other quality improvement initiatives. Participates in medical record abstraction for various HEDIS efforts and other quality improvement project reviews. Performs Patient Service Representative-Integrated duties as needed.
QUALIFICATIONS
Experience in a medical office, human service organization, or related field. Degree or certification may be substituted for experience. Has excellent interpersonal, verbal, and written communication skills. Skilled with computers and computer applications; preferred skill with health information technology systems. Gives attention to detail and has organizational and planning ability. Detail-oriented, strong problem solving, and research skills, and understands the clinical implication and benefits of data quality. Has a positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems. Working knowledge of NCQA’s HEDIS measures, Patient Centered Medical Home requirements, Promoting Interoperability (Meaningful Use) measures not required but preferred.
