Mobile Physician Services is seeking a full-time licensed Primary Care APRN or PA to join our growing team of house-call medical professionals in the Palmetto/Bradenton/North Sarasota FL area. An active FL medical license is required. A reliable mode of transportation is essential as this position requires frequent travel to patient homes within the Ocala area. The perfect candidate will enjoy being an engaging and positive member of our valued team and should have thorough knowledge and understanding of geriatrics with the ability to identify when a patient needs to be referred to specialized care. This role requires the professionalism of the highest caliber with exemplary communication with both internal and external contacts. Strong attention to detail is required as this position routinely handles patient information in electronic medical records. Personable, service-oriented individuals are encouraged to apply!

The Primary Care APRN/PA’s responsibilities include:

• Conduct annual wellness visits for patients, including new illnesses and follow-up exams.
• Diagnose and develop treatment plans for homebound patients for acute, episodic, or chronic illnesses via analyzing and interpreting medical histories, symptoms, physical findings, or diagnostic imaging.
• Maintain standards of care and professional practice guidelines.
• Provide pain management as needed.
• Collaborate with our team of specialists to develop treatment plans for high-risk patients.
• Partnering with local home health agencies, labs, imaging companies, and vendors.
• Communicate with patients and caregivers regarding self-management of diagnosis.
• Oversee an assigned team of Nurse Practitioners and PAs.
• Respond to patient and caregiver questions in a timely and knowledgeable manner
• Maintain compliance with HIPPA and Health Care regulations.

What your background should look like:

• At least one year of home health experience.
