Public Health Nurse
Job description
This position coordinates the ongoing community assessment and community health improvement plan process; facilitates the Strategic and Quality Improvement Plans; maintains and expands community partnerships, implements and evaluates selected public health services and grant programs for a comprehensive approach to community-based health planning, programming, and preparedness; conducts health communication and education for the City of Wauwatosa Health Department.
The starting pay for this position ranges from ($55,827.20 - $76,544.00 +/- annually) , depending on experience and qualifications. Work Schedule:
Monday through Friday
Some evening and weekend hours based on departmental needs.
The following are normal for the position but shall not be construed as exclusive or inclusive. Other related duties may be required and assigned.
Facilitate, prepare and monitor the community health assessment and community health improvement process.- Collect, analyze, and use quantitative and qualitative indicators and data to educate and mobilize the community, develop health priorities, garner resources, and plan actions to improve public health. Provide ongoing analysis of the community’s health, program planning and program execution.

Build and maintain collaborative partnerships with other city departments, state and federal agencies, medical providers, community-based organizations, advocacy agencies, and academic institutions to coordinate comprehensive, city-wide efforts.

Develop community outreach programs and activities to all community partners, conducts community educational programs, and coordinates public health programming with other departments throughout the City.
- Assist in planning, development and evaluation of grant-funded activities in accordance with grant guidelines. Identify new funding sources and assist in grant writing.
- Create health education content for social media, website, newsletters, and other communications channels. Ensure that communications are culturally and linguistically appropriate and reach communities not traditionally reached through public health channels.
- Work as a team member with other public health staff on a variety of public health programs and activities.
- Inform, influence, and motivate individuals, groups, institutions, and public audiences about important health issues, including but not limited to, disease prevention, health promotion, health care policy, the business of public health, risk communication during public health emergencies, and enhancement of the quality of life and health of individuals within the community.
- Develop and implement strategies to increase integration of health equity into department programs and services.
- Assist with identifying, developing, and advancing policy interventions to address social determinants of health to improve health equity.
- Other duties include communicating programming status through prepared reports, written documents, oral presentations, and electronic means; preparing website and media content; and assigned required department technical reports, including the annual report.

May be required to work a flexible schedule including evening hours and occasional weekend events.
Minimum Requirements

Bachelor’s Degree in public health, health education, public administration, nursing, epidemiology, business administration or a closely related field.
Valid Driver’s License and valid auto insurance must be maintained throughout employment
Additional Qualifications
Two years of experience in community education, program planning and evaluation, or public programming preferred.
- Experience in facilitating groups, community coalitions, focus groups and conducting surveys preferred.
- Knowledge and experience in community health assessment, community health improvement planning; community-based program planning, development, implementation, and evaluation preferred.
- Knowledge and/or experience in best practices, health risks, prevention strategies and education, quality improvement processes, and strategic planning preferred.

Knowledge of grant implementation, evaluation, and report writing preferred.
