We are currently recruiting for a full-time Re-entry Case Manager to work in the Transitional Services department. This critical role conducts pre-release and post-release transitional planning services for women, including transgender women and others who may identify as non-binary or gender-non-conforming. Additionally, the Re-entry Case Manager coordinates with on-island staff to finalize reentry plans and provide referrals for needed services in community; co-facilitates trauma-focused support and cognitive behavioral therapy groups as needed.
Responsibilities will include (but are not limited to):
- Regular site visits (at least 3 times a week) to the Rose M. Singer facility at Rikers Island to conduct initial assessments on new participants and provide follow up to existing participants.
- Conduct WPA-developed screening and intake/assessment to determine program eligibility and identify service needs. As well as conduct Department of Corrections (DOC) pre-release transitional plans.
- Establish and maintain collaborative working relationships with project partners including, but not limited to DOC, Correctional Health Services, and other DOC-contracted service providers on-island to facilitate referrals and access to eligible program participants, implementation of reentry plans, and transportation of participants from Rikers to community.
- Provide trauma-responsive case management services, collaborating with participants to develop a reentry plan based on assessed needs and participant interests.
- Accompany participants to the human resources administration, medical appointments, housing appointments, and any other in field support that they may request.
- Write detailed, individualized character/program enrollment letters when needed (for example, confirmation of program enrollment as required by the courts).
- Collaborate with external providers, within the Community Justice Re-entry Network (CJRN) to ensure immediate stabilization supports are in place for participant in need of housing upon release from incarceration.
- Support participant engagement in services, including referral to and follow-up with external providers in areas including but not limited to health and mental health, substance use treatment, parenting support, and educational programming.
- Maintain casework contact frequency appropriate to participants needs and consistent with WPA’s expectations of individualized, trauma-responsive case management.
- Document all activities with, and on behalf of, enrolled participants in WPA’s electronic database on a weekly basis.
- Provide continuous feedback to the team during regular supervision and staff meetings.
- Conduct other duties as assigned that are reflective of the mission of WPA and the program.
Qualifications and Competencies:
- BSW or comparable human-services related BA strongly preferred; will consider equivalent experience.
- Minimum 2 years of experience in direct service with a vulnerable population.
- Ability to obtain clearance from DOC and willingness to travel to Rikers Island 3x weekly
- Experience working with populations impacted by the criminal legal system.
- Knowledge of trauma-responsive care and harm reduction.
- Experience facilitating groups and working knowledge of NYC housing options and entitlements.
- Experience working with and sensitivity to individuals with substance use and mental health diagnoses.
- Experience working in a prison, jail, or other correctional facility preferred, but not required.
- Strong advocacy skills
- Must be organized, detail oriented and a self-starter with the ability to work autonomously and possess an entrepreneurial drive while flexible enough to work within a team.
- Excellent communication skills – verbal and written.
- Demonstrated ability to handle multiple priorities simultaneously and adapt to changing organizational needs.
- Demonstrated analytic skills and problem-solving capacity.
- Demonstrated ability to meet deadlines and to prioritize multiple tasks appropriately.
- Experience using Microsoft Office applications, including Word, Excel, and Outlook.
- Bilingual English/Spanish a plus.
Position Details:
This is a full-time position Monday to Friday, 9am to 5pm; however, flexibility for infrequent time needed outside of business hours as needed. The Reentry Case Manager reports to the Program Director. Currently, work is performed 3x a week at RMSC on Rikers island, 1 office day at the Brooklyn Office site, and 1 remote day. The ideal candidate will be able to provide his/her own transportation. Reimbursement for certain business travel is included.
Compensation:
The salary for this position is $65,000. WPA offers a competitive benefits package and generous time off policy, including medical, dental and vision insurance; pre-tax parking and transit benefits; and a time off policy including personal days and floating holidays.
