Position is remote but open to residents of the State of Ohio only. Must reside in Ohio as of start date.

is seeking a mission-minded Associate to join us as a full time (37.5 per week) Referral Resource Manager.


SUMMARY

The Referral Resource Manager (RRM) is responsible for the development and management of relations, agreements, and systems with other health care providers, community organizations, and payers within the medical neighborhood that support patients receiving equitable and integrated care to ensure an outstanding patient-centered experience and promote positive health outcomes. Relations, agreements, and systems must be designed to address health equity both within and outside of health care including the social, financial, communication, medical, psychological and logistical barriers to accessing health care. The RRM will collaborate with all teams within Health and Clinical Services including, but not limited to all functions within Care Coordination, Health Centers, Customer Contact Center, and Billing, ensuring PPGOH’s mission of providing high quality, equitable, and patient-centered care, as well as creating an outstanding patient experience are met. Additionally, the RRM will ensure the organization is compliant with fulfilling these duties in line with the parameters set forth in the organization’s Medical Standards and Guidelines and any other regulatory requirements.

WHAT YOU WILL DO

  • The Referral Resource Manager supports the Director of Care Coordination with fostering, developing, coordinating, and maintaining relations and networks across the service area to support PPGOH patients in an equitable way with their abortion care, gender affirming care, gynecologic care, smoking cessation, behavioral health, primary care and mental health needs.
  • The RRM collects and analyzes data from a variety of sources, such as payer member data, to identify opportunities for pertinent resources and community partners including to decrease health disparities and address social determinants of health.
  • Maps available resources relative to the location of our health centers and zip codes of patients to promote equitable access.
  • Supports strategy to increase referrals to PPGOH by identifying, soliciting, cultivating, and managing resources and relationships.
  • Supports development of agreements with providers and external medical neighborhood collaborative activities by identifying opportunities and assisting in setting expectations for information sharing and patient care.
  • Maintains close, positive relationships with existing partners to enhance retention and referral.
  • Supports health center managers, clinicians, staff, other providers, and department heads to maintain and enhance referral relationships.
  • Collaborates with other departments to organize and facilitate high-impact outreach events to educate providers, health care partners, and the community about PPGOH services.
  • This position reports to the Director of Care Coordination and must align strategy with that of the vision for the Care Coordination model. Collaboration and findings through research and tried efforts will inform the Care Coordination model.
  • Regular collaboration with the division including, but not limited to Care Coordination, Patient Navigation, and Centralized Follow-Up functions.
  • Contributes to the measures of success around the care coordination program including, but not limited to, measures on care coordination, patient health outcomes, patient retention, overall patient satisfaction and willingness to recommend PPGOH for care.
  • Reports and analyzes the impact of the program as related to the Social Determinants of Health.

WHAT YOU WILL BRING

  • Bachelor’s degree in public health or related field preferred. 3-4 years of experience in public health, health care, case management, or related field may be substituted in lieu of degree.
  • Must have experience strategically identifying, building, and managing referral networks with an emphasis on health equity.
  • Customer service or patient care experience required.
  • Previous experience working with individuals of a diverse socio-economic background.
  • Exceptional interpersonal, team-building, and external relationship building skills to establish and maintain strong relationships with a diverse group of stakeholders.
  • High level of comfort in dynamic, shifting environments.
  • Strong organizational and communication skills.
  • Must be able to work independently with the ability to prioritize and lead with minimal supervision.
  • This position also requires use of negotiation skills, independent judgment and the ability to work effectively under pressure.
  • This position requires the use of presentation and public speaking skills.
