Position Summary:
Under the direction of the Behavioral Health and Nursing management team, ensures the delivery of patient-centered services in accordance with the San Ysidro Health Model of Care. Utilizing evidence based practice to deliver safe high-quality cost effective care, and improve health outcomes by meeting the population health management needs of the communities served.

Essential Functions of the Job:

• Provides direct patient care, monitors medical condition, consults with physicians, and provides counseling, after care instructions, and health education.
• Screens patients and maintains patient records and charts to ensure an accurate record of the patient’s medical treatment. Prepares written documents as required by department and profession.
• Administers medications by all routes. Prepares, hangs, monitors, and discontinues I.V. fluids. Administers vaccines, applies and reads TB Skin tests. Oversees immunizations to assure compliance with program requirements.
• Per policy, orders specific diagnostic and laboratory tests; reviews patient’s history and communicates findings to the provider.
• Maintains clinic-use medications and equipment orderly and properly labeled. Assesses supply and equipment needs and maintains adequate stock. Directs the requisition, care, and maintenance of inventory, equipment, and supplies including daily and monthly logs.
• Follows SYHealth policies and procedures for efficient department operations, reporting requirements, and maintaining quality assurance program, safety, environmental and infection control standards. Attends meetings as required.
• Integrate internal relations with the clinical staff to continually improve patient satisfaction.
• Assists in coordination of nursing schedule to ensure RN and MA coverage during all hours of operation and coordination of medical services to all patients.
• Will rotate to various SYH clinics and departments, such as Obstetrics, Pediatrics, Adult Medicine and Specialty Care. Will coordinate activities and personnel in assigned units.
• Will participate in staff development training

• Maintains professional growth and development through seminars, workshops and profession affiliations to keep abreast of latest trends and news.
• Utilizes and organizes time efficiently to maximize productivity.
• Works both independently and in a team-oriented, collaborative environment.
• Adapts to shifting priorities, demands and timelines.
• Attends and participates in meetings as required.
• Ability to travel to multiple sites within SYHealth network.
• Oversees immunizations to assure compliance with program requirements.
• Performs other related duties as assigned or requested.

Job Requirements

Education:

• Graduate of an accredited school of nursing.
• Preferred: Bachelor's degree (BSN)

Certifications/Licenses Required:

• Current unencumbered California Registered Nurse licensure
• Current driver's license.
• Current BLS certification for Healthcare Providers

Experience:

• Clinical rotations.
• Preferred: 2 years of nursing experience in medical-surgical or outpatient nursing or specialty area.

Verbal and Written Skills Required to Perform the Job:
Preferred: English/Arabic/Aramaic with fluent written and verbal communication skills.

Technical Knowledge and Skills Required to Perform the Job:
Daily operations of outpatient medical office, OSHA regulations and safety procedures. Knowledge of current nursing practices with strong interpersonal and leadership skills is required. Good computer and mathematical computational skills.

Equipment Used (Computers, phones, vehicles, copy machines and fax machines etc.):
Office equipment such as computers, copiers, telephone, fax machines, etc. Medical equipment includes emergency equipment, defibrillator (AED), nebulizer, etc.

Working Conditions and Physical Requirements:
Frequent prolonged standing/walking. Occasionally lifts, positions, pushes, and/or transfers patients. Occasionally lifts supplies and equipment. Handles emergencies or crisis. Occasionally subject to multiple calls and inquiries. Exposed to unpleasant elements (Accidents, injuries and illness). May work variable hours to include, weeknights and weekends.
