Polaris Group, a member of the Assembly Health family of companies, is a nationally recognized long-term care consulting firm. Our remote MDS program continues to grow and we are in need of a Remote MDS Coordinator. This is a wonderful work from home opportunity with a great support system.
Qualified candidates will have:
- 3 or more years of MDS experience in LTC/SNF
- a Current and valid compact RN
- completed a RAC-CT
