DESCRIPTION:
Ethos seeks an RN who will coordinate per diem nursing assignments with administrative assistant to assure that MassHealth Personal Care Attendant program timelines are met. Conduct initial evaluations with the Occupational Therapist (OTR) for consumers applying for the (PCA) program. Complete re-evaluations annually or upon functional impairment change. Complete the MassHealth PCA Evaluation form according to MassHealth guidelines. Maintain the database so that nursing visits can be scheduled effectively.
DUTIES:
1. Work with designated administrative assistant to verify that per diem nurse assignments are correct and will be completed on a timely basis.
2. Work with the PCA team to evaluate consumers for the PCA program following MassHealth Time-for-Task guidelines.
3. Conduct in-home initial evaluations and re-evaluations that accurately reflect the consumer’s functional abilities and need for physical assistance in performing ADLs and IADLs using the standard MassHealth PCA Evaluation form. Consider individual circumstances when determining the number of hours of physical assistance that a member requires for IADLs.
4. Schedule initial evaluation visits with the OTR.
5. Complete forms electronically using the MassHealth LTSS Provider Portal.
6. Provide support and oversight of per diem nursing staff. Assist program director with hiring as needed.
7. .Notify the PCA team when re-evaluations will be late so that extensions can be requested.
8. Meet with PCA team weekly to confer about pending and current evaluations.
9. Attend and participate in MassHealth meetings, committees and workgroups as needed.
10. Perform other related duties as assigned by supervisor.
QUALIFICATIONS:
1. R.N. from School of Nursing accredited by the National League for Nursing (B.S. preferred). Must have a current valid license to practice nursing in the Commonwealth of Massachusetts.
2. A minimum of two years of clinical experience including one year providing services to persons with disabilities.
3. Ability to work well with other professionals and to communicate effectively.
4. Excellent written and verbal communication skills.
5. Intermediate computer and basic word processing skills highly desirable.
6. Must have acceptable driving record/points and submit annually.
7. Must be able to drive or travel in an efficient manner.
8. Must be able to traverse at least two flights of stairs.
9. Must receive TB clearance .
