Position Summary
The Supervisor of Care Navigation will be responsible for the daily oversight of the Care Navigation team which may include Care Navigators, Care Coordinators, and/or Licensed Clinical Social Workers. The supervisor will work in a multidisciplinary team environment to ensure consistency with interventions geared at supporting members. The supervisor will be accountable for meeting the financial, operational, and quality objectives of the organization within key performance indicators.
Daily activities will include engagement with team members to answer questions, problem-solve, offer coaching opportunities, and support individuals on the team. The position will collaborate with internal and external stakeholders to ensure positive patient outcomes.
Responsibilities
- Serves as a subject matter expert regarding practice standards, quality, workflow, and problem-solving to address client, staff, member, and/or patient needs
- Performs regular call monitoring audits
- Performs Monthly Care Services Process Audits per policy
- Ensure client Performance Guarantees are met
- Provides review and feedback of new and ongoing training content
- Monitors and communicates key performance objectives, productivity expectations, and course corrective actions to staff weekly during 1-1 sessions and team settings where appropriate
- Represents HealthMap Solutions Care Services at internal and external audits
- Participates in recruitment, hiring, and development of staff
- Creates an environment promoting the concept of teamwork, individual contribution, acknowledging and incorporating feedback for positive team morale
- Acts as liaison with other key internal business partners focused on customer satisfaction
- Oversees case management rounds
- Oversees projects as assigned
- Provides Root Cause analysis with solutions
- Models and drives effective change management
- Coach and Mentors Staff for development
- Encourages an environment for positive team morale
- Applies forward-thinking with creative solutions to barriers
- Complies with HIPAA privacy laws and all other federal, state, and local regulations
- Ensures compliance with policies and procedures in agreement with regulatory and accreditation standards, NCQA, CMS
- Willingness to obtain additional RN State licensure as appropriate
- Performs other related duties as assigned
- Complies with company security policies
- Follows all company-defined operational policies and procedures
- Uses multiple technical applications including Word, Excel spreadsheets, electronic calendars, email and other software applications
- Monitors and manages key performance indicators as identified by the organization level and department level
- Builds strong cross-functional relationships with internal departments as well as physician partners and practice staff
- Performs other related duties as assigned
Requirements
- Registered Nurse
- BSN preferred
- Certification in Case Management preferred
- 3 or more years of experience in case management
- Experience in a dialysis center or transplant center preferred
- Experience in customer service is strongly preferred
- Proof of full Covid-19 vaccination
