Are you an LPN or RN, looking for a flexible schedule, who enjoys make connections with your patients? Diamond Home Health Care, Inc is seeking a Home Health Registered Nurse and LPNs to join our team. You create your own schedule!

Responsibilities:

• Provide skilled home health services in Northern Virginia area
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Maintain accurate patient medical records
• Knowledge of OASIS D1 documentation

Qualifications:

• Previous experience in home health(preferred)
• Familiarity with medical software (Kinnser) and equipment
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

This position allows for autonomy, flexibility and one on one care in the privacy of the patients residence.

Job Types: Full-time, Part-time, Contract

Pay: $60.00 - $90.00 per hour
