REPORTS TO: Administrator

NATURE OF WORK: Assisting in program and operational procedures responsible for direct supervision of service provision for the Foundation which supports Individuals with Intellectual/Developmental Disabilities. Employees in this role serve as managers over direct support staff and administrative support staff. Under general supervision, the employee works within general methods and procedures and exercises considerable judgment to adapt and apply the guidelines to specific situations. The work requires knowledge of the policies, procedures, and regulations of developmental disabilities programs, and supervisory techniques, and personnel policies/procedures. The employee functions as a first-line manager. Physical Demands: Work may require frequent weekend and evening work, require an employee to work in adversarial situations, and require an employee to work in an occasional hostile environment.

EDUCATION, TRAINING, AND EXPERIENCE:

Compensation:

Dependent upon experience. Equivalent combinations of education and experience that provide the required knowledge, skills, and abilities will be evaluated on an individual basis.

Preferred Knowledge, Skills, and Abilities, Experience or Professional Qualifications:

Bachelor’s degree in human services, or in psychology, social work, nursing, special education, audiology, speech pathology, recreation therapy, speech therapy, occupational therapy, or physical therapy and 5 plus years’ experience working with adult with intellectual disabilities /developmental disabilities population and mental health population preferred. Shall have experience in working with the population served and in providing the services outlined in the Service Description. Ability to develop and deliver presentations. Ability to conduct training for personnel. Work requires professional written and verbal communication and interpersonal skills. Ability to communicate and interact with officials at all levels of government and to work effectively with a wide range of constituencies in a diverse community. Ability to motivate teams and simultaneously manage several projects.

Minimum Knowledge, Skills, and Abilities, Experience or Professional Qualifications:

High school diploma/equivalent and /or at least 5 -year(s) experience in service delivery to persons with developmental disabilities. Certification in CPR, Standard First Aid, Crisis Prevention Intervention Training, Basic Physical, Tuberculosis Test (Annual), Valid Georgia driver’s license, Criminal History Records Checks, and Travel within the region required.

DUTIES AND RESPONSIBILITIES

1. Expected to provide coverage in times of acute shortages of other direct support staff but continues to be responsible for the primary management duties.

2. Stays updated on FEA policies and procedures, service manuals, and other regulations.

3. Ensures proper labor relations and conditions of employment are maintained.

4. Maintains records, prepares reports, and composes correspondence relative to the work.

5. Liaison between FEA and community agencies / workshop programs.

6. Daily collection and review of time sheets for accuracy and corrections prior to submission to Administrator for payroll processing. Submit employee attendance spreadsheet via email daily no later than 8:30am.

7. Social Activity Manager will monitor all classroom and common area from the hours of 7:45am to 2:30pm in collaboration with operations.

8. Monitoring classroom for staff/individual physical contact, sexual contact, physical aggression, anger issues, sexual language, cursing, inappropriate conversation, inconsistence in curriculum procedures (whether it being followed) appropriate language and tone of staff towards individuals, proper seating and spacing of individuals. Behavioral issues between individuals e.g. physical

9. contact, sexual contact, physical aggression, anger issues, sexual language, cursing and inappropriate conversation in coordination with DDP and Behavior Support Staff.

10. From 11:30am to 12:15pm lunch time observation in the lunchroom to insure Person-Centered practices and safety.

11. Responsible for weekly submission of program reports.

12. Contribute to facility efforts to maintain or improve quality of care through participation in the following:

a. Serve as Chair of Person-Centered Committee

b. Serve as Chair of Human Rights Committee

c. Serve as Member of Quality Assurance Committee and

d. Serve as Member of Behavior Management Committee

Staff:

1. Works closely with the Administration and Staff regarding employee work schedules, disciplinary actions, job performance and personal conduct.

2. Assist with the interviewing, hiring, training employees, planning, assigning, directing work, appraising performance, rewarding, disciplining employees, addressing complaints, and resolving problems in collaboration with administration.

3. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws.

4. Selects and assigns staff, ensuring equal employment opportunity in hiring and promotion.

5. Ensure appropriate staffing ratios

6. Ensure staff in each area are placed with designated peer trainers for shadowing to complete one week of on the job training and assuring daily debriefing and support is provided to new hires

7. Complete 30-day evaluation of all new hires

Compliance:

1. Spends time on a regular basis at the location where services are being performed to ensure policy, procedures and program data are in place and being implemented appropriately.

2. Participates in conducting formal and informal investigations into incidents/allegations of abuse, neglect, exploitation or other circumstances that may present risk to safety and health.

3. Performs various assessments; chart reviews, Instruction observations, mealtime, environmental and other monitoring.

4. Attends quality management team meetings.

5. Ensure process of Continuous Quality Improvement is followed.

a. Formulating and trending monitoring Justifications monthly

b. Assist in accreditation process as needed

Quality Assurance:

1. Person-Centered Surveys are administered bi-annually, including organizing a project team to administer surveys by January 1st and July 1st.

a. Once surveys are complete trends, summarizations, and proposed activities must be submitted to administration by January 30th and July 30th of each year.

2. Chair a Person-Centered Activities committee inclusive of employees, administration, and management members

3. Create an annual trip calendar during committee meetings

4. Review activity guide in morning meetings to set activity expectations to ensure appropriate training and understanding of staff implementation of Person-Centered activities

5. Conducts classroom evaluations to ensure activities are being met and submit trends in staff behavior, classroom challenges, and progress notes are submitted weekly.

Responsibilities Regarding Individuals:

Participates in shared planning by assisting each person in identifying and communicating their requests and needs for services and supports.

1. Participate in Quarterly Summary Outcome Evaluations to ensure Person-Centered Activities are being implemented.

2. Provide observation, monitoring, and intervention for unstable medical and behavioral conditions and report observations to the Administration in writing.

3. Communicate with the family and/or care provider(s) regarding all aspects of individual care

4. Weekly inspect and maintain closets containing activity supplies

5. Participate in “Individual Committees,” ensure minutes are properly recorded and timely provided to Administration attached to the weekly report.

Progress Note:

1. Daily: Ensure that all progress notes are submitted to Program Coordinators no later than 3:00pm Monday thru Friday.

2. Weekly: On Fridays, Program Coordinators should provide progress note feedback regarding documentation errors to the Social Activity Manager.

a. Social Activity Manager will develop a written notice of noncompliance with a detailed report indicating the deficiencies by close of business every Friday from the previous week.

3. Monthly: Social Activity Manager will create a Feedback Report using the written notice of noncompliance and detailed report indicating the deficiencies.

a. This report will then generate a staff supervision meeting to discuss potential solutions and/or reprimand (i.e., a plan of action).

Interventions:

Responds to incidents, usually called on walkie talkie, in absence of Program Director / Administrators including, but not limited to

· Physical intervention, an individual acting out

· Medical intervention

· Available for emergency calls and to assist in crisis intervention in support of individuals served as needed

· Responsible for assisting agency Registered Nurse with staffing to support medication administration

Safety & Infection Control:

Social Activity Manager shall conduct no less than two safety observation tours of high-risk activities like work in confined space to verify that employees are performing the work in accordance with safe practices, company procedures. Practices universal medical precautions by understanding and utilizing personal protective and safety equipment.

· Weekly Meetings. Not less than once a week, will collaborate with Administrative team to conduct a meeting lasting 30 to 45 minutes on specific Safety-related, Policy & Procedures and Accountability topics to be attended by all employees.

· Social Activity Manager shall submit weekly a recommendation to improve Safety & Infection Control for staff to take a more proactive role in their own safety.

Activities Coordination:

The Social Activity Manager serves as the primary leader of the agency support team and is responsible for overseeing the implementation of Person-Centered Activities and aspects of group activity, including but not limited to appearance of leisure materials, cleanliness, and community integration, Individual Clubs and Committees through social integration.

1. Coordinates activities by scheduling work assignments, setting priorities, and directing the work of subordinate employees.

2. Schedules program activities and coordinate the necessary support services such as dietary, recreation, housekeeping, clothing, and transportation.

3. Plans program activities in coordination with DDP to support goal achievement of individuals.

a. Maintains all person-centered activities in compliance with all regulations without deficiencies.

4. Provide Person-Centered activity programs on a monthly basis due by the 15th of the previous month. Proposed activities must be submitted in weekly program report to be reviewed by Executive Team.

5. Collaborate with the interdisciplinary team to identify and implement meaningful group activities with focus on emphasizing Person-Centered practices.

6. Plans and coordinates special holiday events, timely, projected timeline of event details must be submitted 30 days prior to event inclusive of projected budget, time allocations, and staff needed.

7. Obtain necessary equipment and supplies and provide for their accessibility through organized storage:

a. Submit projected budget to administration for proposed activities

b. Foster Family and community support of the activity program through Community Members, We the People and People First

Staff Training:

Trains staff on new and changing programs to ensure correct and timely implementation. Identifies staff development and training needs and ensures that training is obtained in collaboration with administration. Ensuring signature pages and supporting documents of training are readily available for Compliance for review.

TRAINING

First 60 days from date of hire it is the responsibility of Administration ensure the following training is complete.

· Person-Centered values, principles and approaches;

· A holistic approach for providing care, supports and services for the individual;

· Medical, physical, behavioral and social needs and characteristics of the individuals served;

· Human Rights and Responsibilities (*);

· Promoting positive, appropriate and responsive relationships with persons served, their families and stakeholders;

· The utilization of:

· Communication Skills (*);

· Behavioral Support and Crisis Intervention techniques that are nationally benchmarked techniques for safe utilization of emergency interventions of last resort (*); and

· The Georgia Crisis Response System (GCRS) to access crisis services.

· Ethnic, and Cultural Diversity Policies;

· Fire safety (*);

· Emergency and disaster plans and procedures (*);

· Techniques of Standard Precautions to include:

· Preventative measures to minimize risk of infectious disease transmission;

· Use of Personal Protection Equipment (PPE);

· Sharps Safety (with sharp containers disposed of according to state and local regulated medical waste rules);

· Environmental Controls for cleaning and disinfecting work surfaces;

· Skills Guides for handwashing, cleaning up spills, gloves use, and what to do with contaminated supplies;

· Respiratory Hygiene/Cough Etiquettes for cough, congestion, runny nose or increase production of respiratory secretions; and

· Approaches to individual education to include incident reporting and follow-up.

· First aid and safety;

· BCLS including both written and hands on competency training is required;

· Specific individual medications and their side effects (*);

· Suicide Prevention Skills Training (such as AIM, QPRP);

· Ethics and Corporate Compliance training is evident.

A minimum of 16 hours of training must be completed annually to include the trainings noted below:

· Human Rights and Responsibilities

· Communication Skills

· Behavioral Support and Crisis Intervention techniques to de-escalate challenging and unsafe behaviors (*)

· Nationally benchmarked techniques for safe utilization of emergency interventions of last resort (if such techniques are permitted in the purview of the organization) and

· Fire safety

· Emergency and disaster plans and procedures

· Specific individual medications and their side effects

· Training provided on proper body mechanics for lifting/transferring/positioning as a basic requirement to assist in performing activities of daily living safely. In addition, prior to working with each individual, training on the individualized specific condition as written in each individual’s care plan protocol should be conducted, if applicable. (*)

· Annual requirements include but are not limited to: Abuse and Neglect training, Blood Borne Pathogens, Confidentiality agreement, Tardive Dyskinesia, Seizure training and others on an as needed basis

· Bi-annual requirements include but are not limited to CPR/First Aid & TB Test

Confidentiality:

1. Ensures confidentiality regarding sensitive material including employee and service user’s individual rights to privacy, and protected health information.

2. NEVER discuss the individuals or his/her program with anyone outside of the family or other team members that also work with the individuals.

3. Never discuss an employee’s performance or behavior to another employee. The performance of each employee on a team shall be held in the strictest of confidence with feedback provided in a professional and confidential manner upon approval of administration.

4. Issues concerning the individual and his/her family should be kept to self unless it concerns the individual’s welfare in which case, that incident should be reported immediately to the Administration. Alleged abuse and neglect situations must also be documented on a Critical Incident Report (CIR) and faxed into the appropriate state agency for review within 24 hours of discovery.

5. Salary and/or hourly pay rates are confidential and cannot be discussed amongst employees. Failure to abide this policy will result in immediate termination.
