Senior LIFE Harrisburg is Seeking an MSW to Join The Team!
Full Time - Bilingual Preferred
Daylight - No Weekends, No Evenings, No On-Call
Senior LIFE is changing the way America sees eldercare, and needs passionate, hardworking, innovative and versatile professionals to join its fast-growing and groundbreaking interdisciplinary care teams. At the center of this integrated approach is comprehensive support of members’ emotional and social well-being, with the LIFE Center Social Worker overseeing the successful development and implementation of many of these integral tasks.
Senior LIFE is committed to enhancing the ability of seniors to stay at home and live safely in the community as long as possible by coordinating comprehensive healthcare and supportive services for our members.
Senior LIFE is an innovative program that uses a team approach to provide seniors with medical, social, nutritional and rehabilitation services. These services are provided at the Senior LIFE Center and are accompanied with in-home nursing care, personal care, and other homecare services as needed. Senior LIFE makes it possible for seniors to remain at home, enjoying the comfort and reassurance of familiar surroundings while receiving the care they need.
Senior LIFE is a Medicare Program of All-Inclusive Care for the Elderly (PACE) operating as a LIFE (Living Independence for the Elderly) program in the state of Pennsylvania.
The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities, including:
- Overseeing direct social work case management services for all LIFE Center members
- Providing social service support including screenings, counseling and external referrals, as needed
- Reevaluating and updating Plans of Care based on change in members’ status, emergent conditions and other relevant circumstances, then communicating changes to all relevant stakeholders (i.e. interdisciplinary teams, families, caregivers)
- Completing ongoing periodic assessments to ensure a thorough evolution of member care, in keeping with changing needs and requests
- Documenting all visits, including deviations from approved care plans and prescribed intervention techniques
- Documenting member medical records and maintaining electronic case management assessments for immediate and future use
- Identifying development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then developing and implementing these improvements
- Counseling families in both individual and group settings, and leading relevant activities to educate these care participants
- Visiting in-home members, assessing needs and developing/implementing comprehensive speech/language care plans accordingly
- General administrative duties, including completing and submitting 1768 documents to required agencies within 48 hours
Due to the hands-on nature of this role, the ability to develop and foster positive, caring, productive relationships with elderly members and their families is essential, as is the desire to work in a unique environment with a unique, integrated approach to care services. Social Workers will work in-tandem with a variety of other internal and external care providers to ensure the best possible care and quality of life for members and, as such, increased sensitivities, empathy and discretion is required. Applicants must have superior communication and analytical thinking skills, with the ability to efficiently and effectively troubleshoot as needs arise.
A Master’s Degree from an accredited School of Social Work is required. Candidates must also have one year of experience working with the frail or elderly population within a healthcare setting. Experience with the Medicaid application process a plus!
