A DAY IN THE LIFE OF A COMMUNITY LIVING SUPPORT SPECIALIST
In this position, you will provide administrative and organizational support to the Return to Community team as well as provide support to older adults seeking to live independently with the services they need. You will be supporting clients and their caregivers by providing resource location assistance and education. You will research home- and community-based service options and comply with specific protocols and documentation requirements.
QUALIFICATIONS
1 year of experience with case management or care coordination in a long-term care or community setting
- Compliance with all state licensure requirements
- Proficiency with Microsoft Office
- Heart for working with the elderly
A bachelor's degree in social work or nursing would be strongly preferred. Do you have excellent communication and interpersonal skills? Are you highly organized and detail-oriented? Do you have strong problem-solving skills? Can you successfully work independently and alongside a team? Are you great at time management? If yes, you may be perfect for this position!
WORK SCHEDULE FOR COMMUNITY LIVING SUPPORT SPECIALIST
This position is hybrid, working two days in the office and three days from home, Monday - Friday, 8:00 AM - 4:30 PM
