Elite Source Health is looking for a Travel or Local Home Health LPN (Licensed Practical Nurse) to fulfill some of our various contract roles in the Chicago area. If you are a Travel LPN(Licensed Practical Nurse) contracts with Elite Source include a housing stipend, Meals/Incidentals stipends, travel bonus, weekly pay, and other reimbursements.

Requirements:

• Successful completion of LPN Program
• Active LPN (Licensed Practical Nurse) license
• Must be able to become computer/tablet proficient with training
• Capable to handle traveling

Job Description:

• Perform home health LPN duties in patients homes
• Radius will vary from state, city and home health client but on average is 50 miles per day
• Tablet or laptop will be provided to LPN
• Case load and note requirements will vary per home health facility
