Young Adult Case Manager: TIP Program
Job description
Join an expanding team and program to meet the needs of young adults with mental health diagnoses. Access Services is hiring Young Adult Case Managers who will join our Transition to Independence Process (TIP) program serving across Lehigh and Northampton Counties, with an office based in Bethlehem, PA.
What You'll Do:
As a Young Adult Case Manager, you will join an elite team dedicated to improving the lives of young adults aged 16-26 in your community. The TIP Model is an evidence-based model that engages youth through person-centered, strength-based planning to aid them in setting and achieving goals within the 5 domain areas of employment, education, living situation, community life functioning, and personal effectiveness and wellbeing. As Young Adult Case Manager, you will meet youth both one on one and in group formats in a variety of settings in the community. You will maintain a caseload of 15 youth while working collaborating with team members to meet the needs of all individuals in the program. Each Young Adult Case Manager will walk alongside and empower youth as they transition to adulthood, build relationships, gain knowledge of resources, and develop resilience.
Work Schedule: Flexible 40 hours Monday-Friday, with a schedule YOU create, provided all program meetings and responsibilities are met as directed.
Requirements
We're looking for Individuals with these qualifications:
- Passionate about helping their community, skilled at engaging diverse populations, comfortable navigating community settings and resources.
- Experienced working in the mental health field, and/or with transition age youth.
- Computer proficiency
- Excellent verbal and written communication skills.
- Valid license with 2+ years of driving experience with no suspensions, reliable vehicle, and car insurance (required)
Education:
- Bachelor’s degree, which includes or is supplemented by at least 12 college credits in Human Services, Psychology, Social Work, Sociology, or a related field of study.
OR
- A High-School Diploma AND 12-semseter credit hours in sociology, social welfare, psychology, gerontology, or other social sciences partnered AND at least 2 years of work experience providing direct care to people with mental health needs.
