About This Access Center Nurse Position

HopeHealth is seeking an experienced Registered Nurse for its HopeHealth Hospice referral center based in Providence, RI. The RN Referral Center position acts as a customer service liaison between referral sources, care providers, professional staff, patients and families to ensure the smooth and timely admission of patients onto our hospice service.

This is a part-time, 24hrs per week, position that is eligible for benefits. The shift for this position is 8am-4:30pm or 9:30am-6pm.


What will you do?

  • Demonstrates commitment to patient care utilizing outstanding nursing assessment and communication skills throughout the entire referral and admissions process.
  • Schedules visits for referred patients within required timeframe. Provides follow up on pending patients and non-admissions. Communicates prospective patient updates to appropriate referral sources.
  • Helps resolve referral and admissions scheduling issues and customer concerns through direct communication with the patients, referral sources, and Manager of Access.
  • Provides clinical follow up with referral sources on upcoming discharges from hospitals.
