Advanced Practice Nurse- Primary Care

Location: Wenonah, NJ (Camden Area)

Shift: Full-Time, 40 hour work week, Monday through Friday.

Summary:
According to prescribed policies and procedures of the organization including all applicable state and federal regulations and under the general supervision and in conjunction with the collaborative physician, the Nurse Practitioner is responsible for performing patient medical examinations and primary care visits. Documents visits in the patients clinical record. Provides medical care services and primary care intervention for patients.

Requirements:

  • Masters Degree in nursing from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education

OR Masters Degree in nursing and completed a post-masters program that focuses on an advanced practice nursing specialty from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education.

  • 2+ years experience as a Nurse Practitioner.
  • Must have understanding of the hospice program of care.
  • Registered Nurse licensed in the State of Pennsylvania.
  • Currently certified as a Nurse Practitioner
  • Valid drivers license and reliable transportation that is insured in accordance with Pennsylvania requirements.

Responsibilities:

  • Performs medical examinations and patient home visits based upon patient need.
  • Assist in managing the patients symptoms including prescribing medication or care by coordinating with referring providers.
  • Able to exercise independent judgement, critically analyze situations, and make recommendations using appropriate problem-solving skills.
  • Able to function effectively as a member of a team.
  • Must have excellent interpersonal skills and the ability to work and communicate within all levels of the organization.
  • Effectively communicates in interpersonal, professional and public relationships.
  • Participates in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.
  • Provide the Services in timely and professional manner and in accordance with; (i) the bylaws and operational policies and procedures of Primary Care, including without limitation, its Compliance Program; (ii) Primary Cares written policies and procedures including those related to billing submission; (iii) local, state and federal laws and regulations
  • Demonstrates and maintains a positive, professional manner, and manages time efficiently and effectively.
  • Develops and maintains effective and positive relationships with the interdisciplinary group, other professionals and physicians in community.
  • Must have an interest in and a sympathetic, compassionate attitude toward caring for the patient.
  • Must maintain a professional attitude and demeanor and is able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others.
  • Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other healthcare team members and reflect concern for the well-being of patients and employees.
  • Must interact with all staff in a positive fashion supporting the mission and vision of the organization.
