Position Summary: The Agricultural Community Outreach Specialist provides activities and programs that improve the health and overall quality of life of community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CSC into the community and engaging residents in improved individual health. Duties include door-to-door outreach, community/neighborhood canvassing; successful medical home and social service referral; Increasing public education and awareness; the accurate reporting of information and data; positive public relations and integration of community services into CSC’s current delivery of care.

Duties/Responsibilities:

  • Make periodic visits to farms, labor camps/campsites located in CareSouth Carolina areas of service, identifying health problems and concerns of migratory and seasonal farm workers and other direct contacts with individuals in the agricultural farm work community.
  • Identifies individuals and their family in need of health care and provides referrals to CareSouth Carolina or another appropriate health care provider.
  • Provides diligent follow-up and tracking of each service referral or service assistance to ensure that individual has achieved access and services have been received.
  • Assists members of the agricultural community with receiving needed services by navigating the service systems and connecting individuals to the right resources; collaborates with other community organizations and programs on behalf of individuals in need of services. To the extent possible, exhausts all available resources to achieve access to medical or other needed services for each individual helped.
  • In collaboration with care teams at CareSouth Carolina centers, provides one-on-one home visits and other community support services to existing CSC patients in need of personal assistance, additional assessment or resource.
  • Coordinates, plans, and participate in agricultural farmworker health events as well as other community events and programs, such as health fairs, festivals, support groups and coordinating council, advisory boards, and other community meetings. Conduct individual health assessments including health screenings and referrals.
  • Provides public relations by representing CareSouth in the community as well as among the agricultural community. Assist with the implementation of programs and services that promote community understanding and identification with CareSouth Carolina and confidence in its services.
  • Collects, assimilates, and reports accurate data on health screenings, referrals, survey results, agricultural farmworker activities and other information required that relates to his or her performance and the operation of CSC community services.
  • Assists with the development and implementation of all other department goals and objectives as required. Actively participates and is involved in Quality projects, develops PDSAs and is an employee that promotes change concepts within their position and with other positions as a team member at CSC.
  • Completes Relias trainings and signs off on PowerDMS documents in a timely fashion.
  • Willingly assists with other duties as requested.

Required Skills/Abilities:

  • MUST read, write, and speak Spanish fluently
  • Ability to assess training needs and objectives.
  • Strong interpersonal and communication skills and the ability to work effectively with a range of constituencies in a diverse community.
  • A valid driver’s license required.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or diagram or schedule form.
  • Ability to work independently and handle problems involving concrete variables in standardized situations.

Education and Experience:

High School Diploma

One (1) to three (3) years experience in field for related field

Proof of full vaccination for COVID-19 must be presented prior to beginning work with CareSouth Carolina. Full vaccination is receipt of both doses of the Moderna or Pfizer vaccine or one dose of the Johnson & Johnson vaccine.
