At Elevation Health Services, our goal is to break down the barriers between growing behavioral health needs of justice-involved clients and accessibility to proper behavioral health services by offering clients a tele-health option.

We are seeking an experienced Behavioral Health Case Manager that has a passion for providing the highest quality care despite any barriers while creating partnerships with community agencies to create long term success for clients.

GENERAL DESCRIPTION OF RESPONSIBILITIES:

The case manager is responsible for assisting clients throughout the recovery process, helping them gain skills, access resources, and regain the confidence to succeed in society and to gain access to community services. The case manager is responsible for utilizing an integrated case management model that includes assessment, planning, implementation, care coordination, and monitoring. Case Managers may serve various populations such as GMH, criminal justice, substance abuse, etc.

Responsibilities:

● Establish open lines of communication for individuals with mental health or emotional concerns

● Offer assistance to individuals with substance abuse issues

● Working collaboratively with client to develop and update treatment goals

● Provide clients with resources as requested

● Collaborate with additional treatment providers, placement agencies, and probation officers

● Coordinate care with client’s assigned EHS individual counselor and group facilitator

● Ensure all documentation is completed in a timely manner

Qualifications:

● Bachelor's degree preferred

● Previous experience in mental health counseling or other related fields

● Proficiency with computer and electronic devices required

● Familiarity with community mental health resources

● Excellent written and verbal communications skills

● Ability to build rapport with clients

● Strong knowledge of wellness, recovery and self-help

● Bilingual Spanish preferred

Position is hybrid. After the 30 day training period you are eligible to work from home a couple days per week.

Must be available to work in our Phoenix office during the training period.

Sign-on Bonus: $500 if you have a Bachelor's Degree; $750 if you have a Master's Degree.

Job Type: Full-time

Pay: $18.00 - $24.00 per hour
