Job Description:
Position Summary: This member of the multidisciplinary team has responsibility to carry out rehabilitation and support functions and assists in treatment under the clinical supervision of staff with professional degrees.
Minimum Qualifications: High school graduate/equivalent. Sound judgement and a genuine interest in helping others. Valid Florida Driver's License with a safe driving record and current auto insurance.
Preferred Qualification: Previous working knowledge in healthcare setting. Knowledge of local community resources.
Special Skills, Knowledge and Abilities: Good written and verbal communication skills, well organized, computer knowledge

Essential Functions/Job Duties:
Develop therapeutic rapport with clients and demonstrate a recovery oriented approach. Facilitate individual and group session’s aid in development of therapeutic relationships. Provide case management services to an assigned group of individuals. Assume primary responsibility for assessing family strengths, challenges, and needs. Obtains collateral information and collaborates with clients’ families, support systems, outside providers and involved agencies regarding progress in treatment and continuity of care. Demonstrate knowledge of community resources to assist individuals in accessing and learning about related community resources and ancillary services. Referrals – Complete, document and follow up on referrals including collateral contacts, discharges, etc. Other Related Duties – Peer reviews, documentation in clinical chart, consultation, treatment team meetings, collaborate with community partners, adhere to departmental and agency policies, procedure, and processes, state reporting, etc. Provides assistance and supervision to clients in their activities of daily living. Provides social interaction opportunities for clients. Basic computer skills – document progress notes. Supports the team/department with tasks as needed and assigned by Team Leader.
Work Environment/Physical Demands: Works in well-lit areas. Sits, stands, bends, lifts and moves intermittently during work hours. Is subject to frequent interruptions. Is involved with clients, residents, family members, personnel, visitors, government agencies/personnel, etc., in a behavioral health setting. Physical strength necessary to implement interventions. The noise level in the work environment is usually moderate to occasionally loud.
Position Type, Expected Hours of Work and Travel: This is a full-time position, and the expected work hours are 40 hours per week, Monday - Friday - 8:30-5:00. Some early morning, evening and weekend hours when needed. Travel is required.
