The BH Link is searching for enthusiastic professionals who are looking to make a positive impact in the Rhode Island community! The BH Link is a 24/7 triage facility providing assessment and referral services for adults experiencing mental health and/or substance use crises. The role of the Care Coordination Specialist will be to constantly coordinate with the BH Link team to determine and address program needs, transitioning seamlessly between multiple roles.

Roles and Responsibilities:

  • Working on site and throughout the various areas and roles of the BH Link program.
  • Attending all required staff meetings and trainings.
  • Completing all required documentation.
  • Practicing effective communication and coordination with the rest of BH Links clinical team.
  • Engaging with clients in a warm and welcoming manner through the continuity of care.
  • Working with program supervisors to identify areas of need within the program throughout the shift, attending to a number of responsibilities, such as:
  • Screening client walk-ins to the BH Link Triage facility.
  • Assisting clients in the completion of intake paperwork.
  • Facilitating client belonging checks and securing items in waiting room lockers as appropriate.
  • Conducting routinely scheduled facility safety checks.
  • Periodically engaging with clients in the facility to ensure safety and provide assistance as needed.
  • Facilitating transportation of clients to and from the BH Link facility.
  • Maintaining facility logs.
  • Assisting in the organization and record keeping of client charts.
  • Calling local facilities to determine current bed status and availability.
  • Coordinating callbacks to clients and providers for the purpose of care coordination and follow-up.
  • Securing insurance authorizations for clients being referred from BH Link.
  • Engaging in strategic outreach to community agencies.
  • Answering calls that come into BH Links Crisis Call Center, including 414-LINK calls and calls from the National Suicide Lifeline.
  • Documenting calls and other client contacts.
  • Helping to coordinate call center follow-up.
  • Participating in program and agency trainings/meetings as required.

Experience and Education:

  • Bachelors degree with 1 year of direct experience working in the human services field.
  • Associates degree with 2 years of direct experience working in the human services field.
  • High School/GED with 3 years of relevant experience in the human services field.
