Join a world-class academic healthcare system, UChicago Medicine, as the Director of Care Coordination responsible for planning, organizing and directing all activities relating to pediatric and outpatient Social Work and Case Management teams. The Director helps set strategic goals for the organization around patient throughput, care coordination across the continuum, and a post-acute strategy, and facilitates work groups charged with implementing specific initiatives around care coordination such as capacity creation.


The Director of Care Coordination will be responsible for providing direction and management of pediatric inpatient and adult & pediatric outpatient care management operations including direct oversite of case managers and social workers. In this role, you will oversee the day to day functioning of the teams through quality assurance, problem solving, recruitment, evaluation of staff competency and performance and maintenance of department budget. The Director is responsible for developing systems and processes, evaluating and ensuring that hospital resources are used appropriately and effectively. You will be responsible for setting the operating and capital budget for the teams and overseeing the employee/human resource related matters. The Director of Care Coordination is expected to develop a working partnership with leadership personnel throughout the organization to promote effective integration of patient/family care needs across the care continuum.


Who you are:

An organized and strategic leader with:

  • Bachelors in Nursing program or Master’s Degree from Accredited Social Work
  • Strong clinical background with ten years experience in healthcare
  • Demonstrated knowledge and experience in project management, effective managerial skills and principles surrounding quality improvement and utilization management
  • Strong computer skills (MS Office, Word, Excel) and experience with data management
  • Effective communication style and a hands-on, teambuilder mindset
  • Current working knowledge of case management, social work, discharge planning, utilization review, bed access, and staffing resource office


What you’ll do as the Director of Care Coordination at UChicago Medicine:

  • Formulate, implement, and evaluate goals and priorities which are consistent with UChicago Medicine’s strategic initiatives.
  • Lead a high-performance team of “system thinkers” who incorporate leadership principles and vision in performing the functions of social work, case management, and hospital access.
  • Assure that variances with care are identified and managed appropriately.
  • Understand changes in health care delivery and function effectively within health care systems.
  • Support and drive new ideas and technologies that create efficiency in the delivery of care at UChicago Medicine.
  • Contribute to the development of administrative policies and clinical policies and procedures.
  • Support strategy for a post-acute care network.
  • Establish relationships with other health care professionals and community groups and coordinate the services of the department relating to other specified agencies.
  • Facilitate and participate in UChicago Medicine committees and task forces to support Care Coordination and overall initiatives aimed at providing excellence in patient care.
  • Understand and comply with standards of regulatory and accrediting agencies.
  • Demonstrate awareness of, and compliance with hospital policies and procedures.
  • Manage budget within established guidelines.
  • Collaborate with hospital departments to effectively monitor the utilization of resources.
  • Plan for long and short-term provision of human resources, supplies, and equipment to meet the departments’ obligations and goals.
  • Oversee the assignments of agency and staff through the Staffing Resource Office and assure efficiencies and appropriate use of resources.
  • Use a broad theoretical knowledge base to establish standards consistent with best practice.
  • Provide clinical direction to Case Managers and Social Workers.
  • Ensure that supportive patient-centered services are delivered to patients, their families and to members of the medical center's community in a timely manner by providing well-trained, dedicated staff who are available both during normal business hours and after hours (by phone or on site)
  • Recognize and identify issues related to patient care standards; develop and implement solutions.
  • Remain available to support staff in emergency and crisis situations by being an active, experienced problem-solver.
  • Provide an environment which encourages staff to accept responsibility for learning as part of the development of their own role.
  • Participate in planning and evaluation of educational programs to meet departmental needs and promotes the development of staff at all levels.
  • Oversee the education of physicians, managers, staff, patients and families related to the care coordination process by constantly identifying future needs of current customers and/or identifying potential new customers.
  • Maintain awareness and knowledge of current clinical issues or advances relevant to departmental decision-making and functioning.
  • Participate in regional and national professional meetings, representing the department.
