JOB DESCRIPTION
Job Title: Case Manager/RIDE UNITED
Reports to: Community Impact Director
Classification: Non-exempt PT (10 hrs/week)
Supervises Others: No
SUMMARY:
Under the supervision of the Program Manager for RIDE UNITED, United Way of Madison County’s transportation gap filling initiative, the Case Manager will be responsible for the timely and accurate intake/review of applicants, approval/denial, and ongoing case management for the long-term portion of the RIDE UNITED program (known internally as Lyft Pass).
The ideal candidate is someone who would like to supplement their income or is retired or otherwise prefers a part-time position with the qualifications necessary to fill this position. This is NOT a full-time position.
This person must be empathetic and have abilities to assist and empower those encountered within the scope of the RIDE UNITED PROGRAM, is computer savvy (strong Microsoft Suite skills and use of case management software) and has a degree in social work or a related field (see next page) OR at least five years of experience in case management). He/she must also have strong communication skills to include competency in interacting with clients via phone.
The Case Manager will be the primary staff person responsible for the application process and ongoing case management for Lyft Pass/RIDE UNITED clients throughout their time in the program, monitoring required progress and providing additional resources as necessary and possible. The Case Manager will also be responsible for drafting and fine-tuning procedures related to this new position.
QUALIFICATIONS:
- Case Management Experience (degree in psychology, human services, healthcare, social work or another related field or 5 years of significant social services case management)
- Communicates effectively and courteously with all contacts—internal and external.
- Works well with a diverse team of people

- Ability to be highly organized and detail oriented in a fast-paced changing team environment
- Ability to learn new concepts quickly and adapt to changing environment.
- Ability follow-through effectively; flexibility to make adjustments as needed
- Performs duties with accuracy and independence, some remote work may be possible after initial orientation period and assessment.
- Ability to accept feedback and apply it to work task improvement.

REQUIRED COMPUTER & EQUIPMENT SKILLS:
- Has good general knowledge of computer operations; uses a keyboard and calculator proficiently and with a high degree of accuracy.
- Uses Microsoft Office Word, Excel, and PowerPoint as well as Internet and email programs proficiently.
- Proficiency in Case Management Database Software
- Uses typical office equipment (i.e., phone, copier, etc.)
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides case management within the RIDE UNITED program that is safe, timely, effective, efficient, equitable, and client-centered.
- Conducts initial comprehensive assessments for client needs as it relates to transportation and their ability to meet the requirements/criteria to participate in RIDE UNITED.
- Evaluates client needs to provide a personalized plan
- Provides proper education on risk prevention to patients, their families and support groups
- Ensures clients’ compliance with agree-upon plans
- Helps clients make informed decisions by acting as advocates
- Maintains current knowledge of RIDE UNITED program in Madison County and Lyft online portal for issuing rides/credits to approved applicants
- Daily organizing workload to meet project timelines and deadlines. Minimizes time spent on personal calls and matters. Complies with organizational and department policies and procedures.
- Participates in meetings as requested
- Meets all applicable safety requirements for the position and work environment including prompt injury reporting.
- Meets attendance and punctuality requirements, reporting time and attendance accurately.

DATA MANAGEMENT:
- Collects information and data from clients to assess eligibility and documents same; analyzes data for monthly evaluation of program.
EDUCATION AND/OR EXPERIENCE:
- Social Work Degree (Bachelors or Masters) required or 5 years’ experience in case management
- Good communication skills (reading, writing, speaking, listening, and understanding) including active listening skills to accomplish the tasks assigned.
- Experience supporting diverse racial and ethnic backgrounds with regards to social services, health, and cultural needs preferred.
PHYSICAL JOB REQUIREMENTS:
- Ability to be flexible in limited space, using public spaces or accessible offices within United Way or working remotely. May need to sit for extended periods of time within your daily shift at a computer station or work desk.
- Uses hands and fingers to operate computers and office equipment for up to 2-3 hours each day
- Requires clear vision at 20 inches or less with or without corrective lenses or adaptive technology, and vision sufficient to use equipment. Color vision required.
- Hearing and speech within normal ranges and sufficient for clear communication face to face and on the telephone or via use of adaptive technology.
