Service Coordinator – Continuous Residential Support Program (CRS) - $17.25/Hr

Closing date: 05/15/2023

FT 40 Hr/wk Assists adults with intellectual disabilities to achieve maximum levels of autonomy and high quality of life. Supports and collaborates with Program Participants to establish and reach goals within the Individual Plan for both the residential setting and the community. Responsibilities include assistance with budgeting, employment and educational counseling, health and wellness, medical and psychiatric services including medication management, transportation, and ADL skill development (examples include cooking, grocery shopping, menu planning, and laundry).

Hours: Sat - Wed, 3:00 pm - 11:00 pm

Duties and Responsibilities:

  • Assists Program Participants with engagement in recreational, social, and community events
  • Assists Program Participants in performing daily living skills and meeting daily living needs such as shopping, laundry, housekeeping, transportation, etc.
  • Monitors Program Participant’s physical and mental health and report relevant information to the Director/Coordinator
  • Strictly adheres to DDS Forensic Program requirements including line of sight requirements as necessary
  • Provides seclusion and restraint support when absolutely necessary to protect health and welfare of individual served and only as required in Agency contracts
  • Strictly adheres to DDS Forensic Program requirements
  • Implements program and agency policy and procedures
  • Participates in the service plan process
  • Provides rehabilitation, basic support, case management, and crisis interventions per client service plan
  • Provides required documentation of services
  • Reports on residential status to Program Director and staff
  • Provides input to team decisions regarding admission, discharge, and counselor assignments
  • Assists Program Participants in maintaining apartments and landlord standards when applicable
  • Assists Program Participants in financial matters as needed
  • Provides transportation related to services when necessary
  • Monitors Program Participant’s compliance to their conditional release order when applicable
  • Required to provide coverage as needed at any of the Agency sites per Agency need
  • Engages in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications, and disseminate as appropriate
  • Provide conditions that maximize safety for co-workers and program participants.
  • Other responsibilities of Reliance Health staff, such as participation in Agency events, adhering to Agency Policies, training requirements etc.
  • Other job-related duties as assigned
  • Performs duties while demonstrating Agency Core Competencies and Values

Education and Experience:

  • Bachelor’s Degree in Human Services or equivalent work experience in the provision of mental health preferred
  • Work experience in the provision of mental health services preferred
  • Valid CT Driver’s License, vehicle registration and insurance
  • Good driving record
