The Certified Nursing Assistant works to provide residents with routine daily nursing care and services in accordance with the resident’s assessments and care plan. We have Full and Part time availability, AM, PM, NOC shifts available at both of our Sheboygan Falls locations as well as our Oostburg location. Great base pay rate, FTE premiums, shift differentials and a sign on bonus of $1,000.00 for part and $2,000.00 for full time.

Essential Functions/Responsibilities:

• Assist residents during meal periods and with nourishments
• Assist residents with daily dental and mouth care
• Assist residents with bath functions (bedbath, tub or shower bath, etc.)
• Assist residents with hair and nail care
• Shave male residents
• Provide eye and ear care, cleaning eyeglasses/hearing aids as instructed
• Keep resident dry, change gown, clothing, linen when it becomes wet or soiled
• Change bed linens; keep linens tight to avoid wrinkles from forming under the resident
• Make beds
• Position bedfast residents in correct and comfortable position
• Assist residents with bowel and bladder functions, take to bathroom, offer bedpan/urinal, portable commode
• Maintain intake and output records as instructed
• Keep incontinent residents clean and dry
• Collect specimens as instructed
• Assist residents in preparing for activities and social programs, church services, parties, visitors
• Assist in transporting residents to/from appointments, activities and social programs
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lift, etc.
• Assist residents to walk with or without self-help devices as instructed
• Perform functional maintenance plans and rehabilitative procedures as instructed
• Change dressings, bandages, binders, etc, as instructed
• Weigh and measure residents as instructed
• Measure and record temperatures, pulse, and respirations as instructed
• Answer resident calls promptly
• Ensure that residents who are unable to call for help are check frequently
• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes
• Document care provided in EMR
• Assist with the care of dying resident and provide post-mortem care as instructed
• Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is maintained at all times
• Attend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.
• Report any observed changes in a resident’s needs and/or condition immediately to a license nurse or manager as appropriate
  

• Maintain certified nursing assistant certification through the state of WI

• Regular attendance, including timely arrival at work, to support positive working relationships with fellow team members and to support quality resident care
• Ability to work in a fast paced rapidly changing environment
• Attention-to-detail and organizational skills required
• Strong teamwork and communication skills are critical
• Ability to lift up to 40 pounds
