Job Description Summary

The Community Outreach Worker: STI Prevention staff member is responsible for providing outreach services and education to aid in providing STI screening, prevention, and treatment. This role is hourly, full-time, and reports to the Community Health Worker Lead.

Transhealth is a new health center dedicated to providing comprehensive gender-affirming clinical care, research, advocacy, and education in partnership with our community. Objectives & Responsibilities

  • Work with our community partner, New North Citizens’ Council (NNCC), to provide outreach services to community members in the Hampden County area
  • Provide services to assigned clients including prevention, education, intervention, one-on-one patient interactions; educate clients on the importance of STI prevention and treatment, including but not limited to education on potential available prevention and treatment options
  • Develop and design the scope of this new project for Transhealth, alongside the Community Health Worker Lead
  • Attend regular partnership meetings with local stakeholders to develop goals for screening, prevention, and treatment
  • Help develop and facilitate community programming around STI screening, prevention, and treatment
  • Assist participants to identify appropriate programs for necessary resources such as detox, mental health, food, clothes, STI, and HIV/AIDS counseling and testing; refer individuals to Transhealth for STI treatment as needed
  • Provide outreach including door-to-door, on the street, and in abandoned buildings and private homes where needed
  • Coordinate vaccination clinics in tandem with the Department of Public Health
  • Maintain sensitive & confidential client information according to HIPAA
  • Develop and maintain detailed records of activities, events, and information obtained
  • Attend educational programs to keep up-to-date on related health issues
  • Perform other duties as assigned

Qualifications & Experience

  • High School Diploma or equivalent
  • Teamwork and initiative: balances responsibilities, open, gives and welcomes feedback, builds positive team spirit, volunteers readily, and seeks increased responsibilities
  • Existing knowledge of STI screening, prevention, and treatment; issues of importance to low income communities, unhoused people, people living with HIV, and/or people experiencing substance use disorders
  • Thorough, and preferably personal and direct knowledge of transgender and gender-diverse lives and experiences
  • Strong knowledge of the local Hampden county community
  • Spanish language skills strongly preferred
  • Shows respect and sensitivity for cultural differences, promotes a harassment-free environment, treats people with respect and upholds agency’s values
  • Access to transportation to allow for regular travel within the region for partnership meetings and outreach; must have valid driver’s license

Must understand the sensitive nature of our organization and facility - confidentiality and discretion are crucial to the safety of our patients and staff.

Physical Demands & Work Environment

Duties may require sitting, standing, bending, and lifting. Position will require use of personal protective equipment. This position will require the use of a valid driver’s license and ability to travel within the region on a regular basis. Must adhere to OSHA regulations. Pay: $20.00 - $22.00 per hour
