Director, Health and Community Development (Health and Community Development)

Labor Grade 133 / Pay Range: $110,820.61 - $171,932.80

SUMMARY
The Director of Health and Community Development is responsible for developing plans for community-based health and wellness programs aimed at the prevention of disease and the promotion of health. Ascension Parish Government’s (APG) vision for this position is as follows:

  • Takes a holistic approach to public health and recognizes that complete, community wellness involves physical, mental, and social wellbeing
  • Approaches public health collaboratively by seeking partnerships in an effort to include all public agencies and private healthcare stakeholders
  • Transitions public healthcare delivery to have a greater presence in the future and a greater focus on preventative measures
  • Addresses the social determents of poor health outcomes and places emphasis on addressing community conditions that lead to poor health outcomes

The Director oversees, tracks, and manages the Public Private Partnerships (PPP) that impact community health and wellness for early childhood education and integrated public/community and behavioral health services. The position has oversight of operations of the APG Public Health and Mental Health Units, which are managed via Cooperative Endeavor Agreement (CEA) by Franciscan Missionaries of Our Lady Health System (FMOLHS). This position also directly supervises the Nurse Supervisor located at the Ascension Parish Jail in Donaldsonville and reports directly to the Chief Administrative Officer (CAO).

MAJOR DUTIES

  • During the terms of all applicable CEAs, discusses and monitors progress and results with the Contractor. These discussions shall include, but not necessarily limited to, ongoing plans for the continuation of the project and any deficiencies noted, including mechanism for resolution of said deficiencies
  • Reviews and analyzes the Contractor’s progress to ensure compliance with services agreed upon, ensuring that activities are completed on a quarterly basis and/or as needed during the course of the agreement as follows: (1) compare progress to the provision of services, goals, and objectives outlined in the contract to determine progress made; (2) contact contractor to secure any missing deliverables; (3) maintain telephone and/or email contact with the Contractor on contract activity and, if necessary, make on-site visits to contractor sites in order to review the progress on, and completion of, contractor’s services to assure that performance goals are achieved, and to verify information when needed
  • Full supervisory/personnel functions for hiring, discipline, training, and performance management for the Nurse Supervisor and staff
  • Plans and develops, in coordination with our PPPs, APG healthcare policies and procedural improvements throughout all related departments including but not limited to Public Health, Mental Health, Communications, and Recreation to impact obesity prevention/intervention, behavioral health, and substance abuse
  • Collects, organizes, analyzes, charts and tracks data related to evaluation of specific projects; generates reports on quantitative and qualitative project outcomes
  • Cultivates and develops media and other community relationships
  • Recruits and develops the Healthy Ascension Board of Directors to be composed of key leaders from healthcare organizations, the business community, and representatives of the not-for-profit community
  • Develops, maintains, and frequently updates the Healthy Ascension website and social media pages
  • Routinely provides reports to the Healthy Ascension Board of Directors and Chief Administrative Officer
  • Seeks additional funding through grants and nonprofit organizations to support overall health and wellness programs across APG
  • Prepares and assists in monthly and yearly financial operating reports to the administration and Parish Council
  • Serves and develops collaborative relationships with the following external regulatory agencies: Families in Need Services (FINS), Housing and Urban Development (HUD), and the Council on Aging
  • Oversees the APG program funds for Mental Health, Public Health, and the Juvenile Justice Fund – Early Childhood Development
  • Any other duties as assigned

MINIMUM EDUCATION AND EXPERIENCE QUALIFICATION REQUIREMENTS

  • Education and Experience: Bachelor’s degree from an accredited 4-year college or university in Health, Business, or Public Administration or a related field of study and (10) years directly related experience in healthcare administration or a related industry field is required, which includes at least (3) years of supervisory/management experience.

*A Master’s degree in a related field is highly preferred.

LICENSES AND CERTIFICATION REQUIREMENTS – CONDITIONS OF EMPLOYMENT

  • The incumbent is subject to a pre-employment physical and drug test with random drug testing thereafter.
