Description
The Family Services Coordinator for HealthySteps provides case management, care coordination and community navigation services to HealthySteps Program patients and families, which are families with children ages 0-3. The Coordinator will participate in planning and implementation of interventions, partner with other community-based organizations, work with families on goal setting, and ensure follow-up and coordination of services. Engage in outreach activities for parents and families in the community. Assist with administrative tasks related to the HealthySteps program.
Qualifications


Minimum Education

Bachelor's Degree In related field preferred (e.g., public health, public policy, psychology, education)

Minimum Work Experience
3 years' experience working collaboratively with multi-disciplinary team; Experience in child health preferred; Experience working with children with social-emotional challenges or developmental delays preferred.

Bilingual (Spanish/English) preferred
.
Required Skills/Knowledge
Knowledge of community-based and healthcare service delivery systems; Ability to think creatively and strategically; Ability to collaborate with multidisciplinary teams; Ability to multi-task and work pro-actively and independently within a fast-paced, dynamic setting; Skill working with culturally diverse patient, staff and community populations; Strong organizational and project management skills; Strong interpersonal, written and oral communication skills; Proficiency in Microsoft Office; Strong ability to document work in medical records.

Functional Accountabilities
Program Coordination

  • Coordinates community-based project(s) in collaboration with external partners

  • Assists in the development of project scope statements, timelines, and deliverables
  • Maintains understanding of advances in early childhood issue area(s) of responsibility and incorporates into work
  • Works with staff and partners to identify and address system barriers
  • Prepares periodic summaries of projects for internal and external audiences
  • Recommend and participate in development of new processes and systems to meet project and team needs

Case Management Activities

  • Identify appropriate resources; make referrals and follow up on referrals.

  • Monitor the progress of care plans.
  • Encourage adherence to plan through telephone contacts and supportive interventions and education.
  • Work with other team members to provide multidisciplinary, comprehensive care to clients.

Administrative Support & Record Maintenance

  • Complete administrative tasks related to patient care as required.

  • Complete administrative tasks related to program monitoring as required.
  • Maintain, log, compile, enter and help analyze data, prepare charts, graphs, and results of studies.
  • Maintain required records, reports and statistics that pertain to potential and existing clients.

Patient/Family Focused Services

  • Meet with supervisor to discuss case material, professional role, and projects to complete.

  • Meet with mental health clinician to review work performed for individual patients and families.
  • Participate in team meetings and departmental staff meetings.
  • Meet any departmental requirements for chart recording, statistics, and other documentation and submits documents in a timely manner.
  • Participate in departmental performance improvement activities.
  • Participate in professional development and trainings as relevant.
