The IEP Coordinator is a certified or licensed individual in one of the job titles below who is responsible for ensuring that IEP recommendations are implemented and that each service provider responsible for implementation of a student’s IEP is aware of his or her IEP responsibilities, including specific accommodations, program modifications, supports and/or services for the student, prior to implementation of such program. Serves as a liaison to the school district Committee on Preschool Special Education. Will admit and discharge students across all preschool sites.
RESPONSIBILITIES:
- Ensures that every student has an IEP, and that each IEP is accurate, reflective of the needs of the student, and focused on return to a less restrictive environment (LRE)
- Monitors compliance with all regulations school district/student related.
- Respond to SD inquiries pertaining to student’s status.
- Address issues related to student attendance with school districts.
- Notifying school districts regarding any allegation of abuse or student needing emergency psychiatric transport.
- Notify school district regarding a student SD transfer and student discharge.
- Follow up with SD personnel regarding discharge date for school age students. STAC end date must match with student’s enrollment/discharge date at ACA.
- Inform and notify SD rep. of student being referred to the school district for an alternate placement.
- Address parental concerns with school district personnel.
- Meeting Facilitation
- Facilitate all student IEP meetings with school district, parents, and families.
- Facilitate semiannual school, district IEP review meetings for out of state students.
- Document set up and review
- Set up new IEPs in Frontline
- Review all Frontline draft annual IEPs and *supporting documents prior to being sent out to school districts. *Supporting documents include – Annual progress report from each provider, annual physical, BIP (if applicable), annual physical, PFF (if applicable), any evaluation reports.
- Review all Frontline quarterly reports across four quarters and supporting documents prior to being sent out to school districts
- Review new admission IEP to ensure programmatic supports aligns with our program structure.
- Coordination and organize all student IEP meetings.
- Coordinate all IEP meetings with school district, parents, and families.
- Invite/inform relevant team members of dates of scheduled IEP meetings
- Alert SD personnel regarding students who are due for reevaluations/annual reviews or reconvenes.
- Coordinate classroom observations for screenings/evaluations/intakes etcs across all preschool sites.
- Coordinates annual and quarterly treatment planning to evaluate IEP goal progress for all students
- Send out annual IEP memo to all service providers prior to the annual meeting with information regarding Turning 5, meetings scheduled, meetings not scheduled and document due dates.
- Send out quarterly IEP memos
- Coordinates all aspects of the preschool admissions process
REQUIREMENTS:
- Certified Special Education Teacher, School or Licensed Psychologist, Social Worker (Licensed or Master’s Level), or Certified Administrator.
