Santa Cruz Community Health (SCCH) began as a women's health collective in 1974 with the mission to improve the health of our patients and the community and advocate the feminist goals of social, political, and economic equality. Now, more than 45 years later, we serve that same mission as a nonprofit Federally Qualified Health Center operating three separate sites: the Santa Cruz Women's Health Center in downtown Santa Cruz serving women and children; the East Cliff Family Health Center in Live Oak, serving everyone; and the Santa Cruz Mountain Health Center providing easier access to care for our patients in the San Lorenzo Valley.

In December 2022, the East Cliff Family Health Center will change locations in Live Oak and move into a brand-new space offering an integrated, state-of-the-art health and housing campus. SCCH will continue to provide medical, behavioral health, and specialty care with a focus on pediatrics. The campus addresses a triple goal of increasing access to healthcare, growing affordable housing, and creating economic opportunity. Partners in the project are Santa Cruz Community Health, Dientes Community Dental Care, and MidPen Housing.
POSITION SUMMARY:
Under direct supervision of the Lead School-based Counselor and Liaison, the Behavioral Health Provider I: School Based will provide direct counseling and clinical case management services to patients of Santa Cruz Community Health who are students within the Live Oak School District. In this role you may also provide crisis intervention support; participate in educational and mental health meetings as part of a multi-disciplinary team; engage in planning and treatment plans with parents, medical providers, mental health professionals and District staff. As a Behavioral Health Provider I, this position may also see patients in the clinic setting as needed.

CORE JOB RESPONSIBILITIES:
- Provide direct counseling and clinical case management services to students K-8th grade
- Provide individual therapy to children and adults in a primary care setting
- Serve as a liaison between the school district and the health center in the coordination of student supportive services
- Consults and collaborates with members of the patient’s care team, around care coordination, treatment plans, goals, and interventions
- Participates in the social, emotional, and behavioral development of students through care coordination
- Creates and implements care plans to reduce barriers of care and address social determinants of health
- Close collaboration with medical providers, internal behavioral health department, and school staff
- Consult with caregivers and school staff regarding the behavioral, social, and emotional needs of students, and recommend appropriate supportive services
- Provides and tracks referrals, to help patients and families make connections to community resources
- Coordinate the implementation of appropriate support or services
- Evaluate clients’ progress and reassess patient goals as needed to improve outcomes
- Provides risk assessment and crisis intervention services as needed
- Participate as a team member in case consults; participate in student specific educational and or mental health meetings as required (e.g., SST and CCT meetings)
- Documents all patient clinical activity and care coordination to professional standards.
- Participates in reflective supervision meetings.
- Participates when appropriate in community-wide school district meetings.

QUALIFICATIONS:
- Master’s degree in social work, counseling or psychology required. Must register with Board of Behavioral Sciences as an Associate. Clinical supervision provided to those pursuing licensure as an LCSW or LMFT.
- Bilingual in English and Spanish required. Bicultural preferred.
- Experience and knowledge about child and adolescent growth and development, parent-child relationships, child health, infant mental health, and family systems.
- Knowledge of and experience of working with elementary and middle school pupils
- Excellent oral and written communication skills.
- Demonstrated commitment to working with underserved populations in a school setting.
- Must be empathic, supportive, and patient.
- Strong motivational interviewing skills.
- Ability to communicate effectively with students, parents, and school staff.
- Ability to work with people of many cultures.
- Ability to take initiative and a willingness to learn.
- Ability to work well both in teams and independently.
- Ability to travel to other school sites/locations.
- Comfortable with use of technology: Electronic Health Record, Teams, Outlook, Excel

SALARY AND BENEFITS:
Salaried, exempt position with a pay range of $81,120 - $96,408 per year. Competitive compensation & benefits package are available to staff working at least 20 hours per week. Paid time off and paid holidays accrue from date of hire. Paid License(s) and CME (fees and days off). Employer subsidized group health, dental, vision and life insurance plans the first of the month after 30 days of employment. Automatic 2% enrollment in an Employer sponsored 401K plan with a 2% retirement match.
