Our Licensed Practical Nurses assist with all (home based) skilled nursing care based off the plan of care for patients located within an Arlington, Falls Church, Great Falls, Reston, Crystal City, Old Town, Tysons territory.

The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide skilled nursing care to patients on an intermittent basis.

Required Qualifications:

• Graduate of an approved school of professional nursing
• Current and valid Virginia LPN License
• Passionate and motivated to provide care within a patient's home.
• Ability to exercise initiative and independent judgment.
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Ability and willingness to travel within a given territory.
• Willingness to work with multiple patients in varying home locations.

Preferred Qualifications:

• Two (2) years' home health nursing experience, preferred.
• Previous experience with Home Care Home Base EMR system strongly preferred.
• Wound Care experience or Certification strongly preferred.
