Rate is $28.50 per hour plus shift differentials (5%-10% depending on shift) for evenings, overnights and weekends.

Responsibilities:

Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served.

Job Responsibilities:

• Administer medications and treatments as prescribed by the physician
• Generate incident, seizure, and illness reports
• Monitor and report health care on daily flow report records
• Instruct direct support professionals in the above, i.e., hygiene, diet, weights, etc.
• Submit written monthly, quarterly, annual nursing/medical progress notes

Required education: Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.

Required license or certification: New York State Licensed Practical Nurse
