The Manager of Navigation Resource Teams provides clinical, staff and operational supervision to three Navigation Programs: the Central Navigation team, Benefits team, and the Mostly Meds Team. They provide additional support for the Navigation Department as assigned by the Director of Navigation. This position also manages homeless prevention, utility assistance, and dental grants.
Essential Duties:
- This position is responsible for program development and operational leadership of Navigation Programs including the Central Navigation, Mostly Meds, and Benefits Teams. This role also supports other management functions within the Navigation Department and on projects as assigned.
- Provides assistance with monitoring referrals in and out of Navigation, oversight of administrative needs and other client communications for the Navigation Programs.
- Monitors and provides oversite and supervision to above teams.
- Administratively manages and oversees the distribution of homeless prevention and other grants including Filling the Gap, Energy Outreach Colorado, and CDBG: Grants total ~$135,000 annually.
- Review clinical records for Navigation clients to assure appropriate treatment and documentation according to all state/federal regulations and current best clinical practices.
- Provides back up supervisory and other Navigation program services as needed.
- Navigation Program staff members work with all program customers including Jefferson Center consumers, staff members and members of the community to assess, link and follow up on identified resources internally and in the community. Empowers customers to access identified resources and assists with navigating systems of care to meet identified outcomes.
- Coordinates with all Jefferson Center teams, to create mutual clinical and administrative workflows.
- Knowledgeable about Jefferson Center and community resources and builds relationships with providers to assist in a seamless referral process. Works to develop effective ways to research, update and communicate these resources to Navigation customers.
- Maintains a positive focus and excellent customer service skills towards consumers, Jefferson Center staff, and the community. Works from a strength based, recovery-based perspective and takes cultural considerations into account when appropriate. Effectively responds to client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, emails, and other requests.
- Documents all contacts according to Jefferson Center and Navigation Program standards while meeting productivity standards as identified by the Program Director
- Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
- Maintains effective interpersonal relationships with consumers, peers, subordinates, upper management, visitors and the general public.
- Ability to multi-task, prioritize concerns, and effectively manage time independently.
- Attends mandatory in-services, compliance with individualized training plan if required.
- Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem-solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor.
- Submits 90% of all SRFs/Progress-to-Date forms within 3 working days (team specific).
- Completes 95% of all paperwork by the due date.
- Oversees team members meeting their required number of consumer service hours or other measures of productivity as determined by Network Director.
- Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.
- Participate in Evidenced Based Practices and other agency initiatives including Trauma Informed Care, Whole Health as well as other best practices.

Education, Knowledge, Skills & Experience Required:
- Master’s Degree (from accredited University/College) in Mental Health Field required.
- Licensed (LCSW,LPC,LMFT,LP) in the State of Colorado required.
- Two or more years of experience preferred
- Consistent evidence of timely and accurate work products required.
Salary Grade 16
- $70,600 - $93,000
- Salary is determined based on years of total relevant experience.
- Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.
**BONUS OPPORTUNITY!! $1,000 bonus paid out on one month anniversary. ** Based on full time (40 hours/week). Prorate if less than 40 hours/week. Must work a minimum of 20 hours/week to be eligible for bonus. Current Jefferson Center employees are not eligible for sign on bonus*
