Position Overview
Ensures that physical therapy services are administered and completed in a timely, accurate, and reliable manner. Provides physical therapy treatments for skilled nursing facility inpatient and/or outpatient services, as well as associated clinical documentation. Maintains specific service delivery standards which address commitment to service, customer satisfaction, ongoing evaluation of customer expectations, integration with quality process, communication and interaction standards, turn-around time and information processing standards, corporate compliance requirements, and physical amenities/environment of care.

Essential Functions:
Essential functions are those tasks, duties and responsibilities that are performed with or without reasonable accommodation.

● Follows specific treatment procedures as directed by the supervising physical therapist, including preparing the patient for treatment, informing and educating the patient in treatment indications and contraindications, performing treatment procedures. Demonstrates knowledge of rehabilitation equipment. May administer standardized, structured tests under the supervision of a physical therapist.

● Maintains accurate records of each patient’s treatment, progress, and/or regression according to the Service Delivery Standard using paper and electronic systems, as necessary. Maintains all necessary records required to meet the criteria established by reimbursement and regulatory agencies. Maintains confidentiality of patient and hospital information.

● Prepares the patient for treatment protocols and performs the necessary treatment as designed. Monitors the patient's response to treatment and records same in the medical record. Reports immediately to the physical therapist and/or physician any untoward events taking place with the patient.

● Collaborates with other professional staff such as physical therapists, and co-treats, as required, assisting the occupational and physical therapist in assessments of patients.

● Establishes and maintains effective working relationship with patients, their families, and other staff personnel. Participates in patient care conferences with other professionals involved in the management of cases. Provides patient/family education, as needed.

● Collaborates with the physical therapist, participates in planning decisions for patient’s post discharge placement. Makes appropriate referrals and follows up to ensure proper placement and programming of patient. Interfaces with vendor to provide needed durable medical equipment.

● Provides direction to departmental aides as related to treatment, Volunteers, PTA Students.

● Maintains necessary CEU’s for licensure and/or specialty certifications and professional organizations.

● Understands MDS3.0, RUG levels and care plans.

● Participates in Care Conferences.

Position Requirements
Education/Experience

  • Minimum Required:

Graduate of an approved Physical Therapist Assistant program or equivalent.

  • Preferred:

Clinical experience in a hospital based setting.

License/Certification

  • Minimum Required:

NYS license or eligibility to be licensed as a Physical Therapist Assistant.
CPR certification within first 3 months of employment.
