Population Health Program Manager

Job Summary
The Health Population Manager develops and maintains performance improvement initiatives and ongoing communication with physicians and administration regarding productivity, quality outcomes, patient satisfaction, appropriate utilization and all other population health initiatives. Directs the application of performance excellence and quality improvement principles and methods to improve organizational performance, including but not limited to: analysis of current performance to determine areas of strength and opportunities for improvement, and comparison with best practices and national standards.
Job Requirements
Experience: 5 years of professional-level experience in healthcare, OR a Master's degree
Education: Bachelor's degree
Special Skills: Proficient in Microsoft applications. Must have strong writing and speaking skills. Demonstrated leadership skills including superior interpersonal/human relations and communication skills - both written and verbal. Excellent problem solving, critical thinking and conflict resolution skills as well as high level organization and facilitation skills.
