Want to join our Team? We are currently looking for a compassionate and dedicated Clinical Team Lead to join our interdisciplinary team. As a Team Lead, you will support the Clinical manager in providing organizational leadership to our interdisciplinary team in the field. You will work with your team’s Social Workers, Chaplains, and CNAs, as well as our Hospice Physicians, to provide the physical, psychosocial, and spiritual aspects of care. This role is a perfect entry point to Management for an experienced RN or Social Worker.

Coverage Area: Charles County Inpatient Care Center
Schedule: Rotating Days 9am - 7:30pm.
Status: Full-time
Salary: Competitive, commensurate with experience, education and credentialing
Main Responsibilities:
- Acts as a resource to employees by assisting with coordination of patient care planning and outcomes. Assists with patient scheduling for nursing visits and revisits.
- Schedules regular interdisciplinary team meetings on a weekly basis and assures collaboration between all disciplines to coordinate patient care planning. Educates employees on changes and/or on new material and assists with precepting new hires.
- Reviews quality and operational reports and data with the teams to improve patient financial and clinical outcomes.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
Your Qualifications:
- Possess a current Maryland nursing (RN) license, or Licensed MSW. Bachelor’s degree in Nursing, Healthcare Administration, or other related field (preferred).
- 2-3 years of nursing experience in a medical/surgical or home care setting OR 2-3 years of clinical social work required; previous hospice experience strongly preferred.
- Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
- Ability to operate the necessary equipment to perform the job, i.e., FAX, computer, copier, telephone system.
- Must be able to lift or move patients in a safe manner. Must be able to lift 50 lbs.
