Valley Village is a nonprofit organization providing services to adults with developmental disabilities. We are currently seeking a Social Worker to work at our Sunland Adult Day Health Center.

This is a full-time non-exempt position. The hours for the position are Monday thru Friday 7:30 am-3:00 pm

Job Summary:

  • Schedule and lead multidisciplinary meetings in conjunction with the program director.
  • Interview and screen all referrals to determine general appropriateness of the perspective participant for the full assessment process and adult day health care participation.
  • Provide referral for persons not appropriate for the program.
  • Provide signed and dated documentation for all services performed the day the service
  • Provide for periodic reevaluation and revision of plan of care.
  • Provide counseling and referral to available community resources.
  • Promote peer-group interaction and relationships through discussion groups.
  • Serve as liaison with the participant’s family and home.
  • Serve as liaison with other community agencies that may be providing services and coordinate all services to a participant and work with these agencies to coordinate all services delivered to the participant to meet the participant’s needs and avoid duplication.
  • Provide discharge planning for participants.
  • Supervise and train assistant social workers.
  • Other duties as assigned

Requirements:

“Social worker” shall be a person who meets one of the following:

(1) The person holds a master’s degree in social work from an accredited school of social work.

(2) The person holds a master’s degree in psychology, gerontology, or counseling from an accredited school and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services.

(3) The person is licensed by the California Board of Behavioral Sciences.

(4) The person holds a bachelor’s degree in social work from an accredited school with two years of experience providing social services in one or more of the fields of aging, health, or long-term care services.

  • At least 3 years of experience working in a licensed health care setting.
  • Must be able to comprehend and communicate in English.
  • Must meet all insurance requirements which include a valid driver’s license with a good driving record, criminal record clearance, etc.
  • Must maintain the highest standards of appearance (personal grooming and dress) and set an appropriate model for the participants.
