Babylon Health is seeking a Field Community Health Worker (CHW) to join the population health team across the Burlington, IA area. As a member of a team of nurses, social workers, registered dietitians, physicians, pharmacists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with complex and/ or chronic conditions. You will connect with Babylon members to assist in improving their health, locating community resources, and assist in coordinating their care. As a Field Community Health Worker the primary work setting is in the community where members are being served. You must be prepared to work from any of the following settings: hospital or provider office, community agency, a patient's home or community setting. The Field CHW will work collaboratively with in-person health settings, assisting with in-person care coordination and the coordination with Babylon services.

What You will Be Doing:

  • This Field CHW will connect with the Babylon members over the phone or in person at locations approved by Babylon.
  • Assists Babylon members in their homes, community, or clinic setting. Communicates to members the purposes of the program and the impact it may have on their wellbeing.
  • Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
  • The Field Community Health Worker will work directly with community agencies, provider offices and member and must adhere to policies and procedures to maintain personal safety, information security, occupational health needs, professionalism and transportation requirements
  • Serve as a liaison/link/intermediary between the provider, health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery
  • Educates member on the proper use of the Emergency Room, and provides information for alternatives. Coaches patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions set by the Babylon Care Team. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Establish and maintain trusting relationships with members on a daily basis to effectively work with an interdisciplinary care team, internal departments, and with community organizations.
  • Document and track ongoing activities, progress to care plan objectives and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Provide care coordination, which may include but not be limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up.
  • Complete Social Determinants of Health (SDOH) evaluations
  • Provides support and advocacy during initial Babylon or community based medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed.
  • Work collaboratively with local health facilities to initiate transition of care programs introducing and educating on Babylon services.
  • Make referrals and connections to community resources to help individuals and families meet social needs.
  • Continuously communicate and expand knowledge of community resources, services or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.
  • Helping patient access community and governmental based service agencies including filling out paperwork with the patient in addition to providing assistance with the Babylon platform
  • Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
  • Comply with reporting, record keeping, and documentation requirements per Company policy.
  • Work collaboratively and participate in integrated care team meetings..

The Experience You Need (Required):

  • Bachelor's Degree or equivalent related experience in the social services field
  • Two or more years in any health setting
  • Experience working with Medicaid/Medicare/Vulnerable Populations
  • Service-minded and customer service focused.
  • Computer proficiency and strong computer literacy
  • Experience with coordinating care, advocating, or navigating on behalf of patients.
  • Effectively leverage resources to create exceptional outcomes, embraces change and constructively resolves barriers and constraints
  • Must demonstrate excellent written, verbal and interpersonal communication skills, including exceptional listening skills.
  • Experience working directly with patients, physicians, physician office staff, healthcare team, etc.
  • Experience with coordinating care, advocating, or navigating on behalf of patients.
  • Able to work independently and as a positive member of a team
  • Able to follow all HIPAA rules by maintaining confidentiality and security of sensitive information
  • Valid driver license, a satisfactory motor vehicle record check, and personal auto insurance with appropriate insurance limits.

Finishing Touches (Preferred):

    • Successful completion of Community Health Worker (CHW) formal training is preferred.
    • Experience working with patients with chronic and behavioral health needs
    • Bilingual (Spanish & English)
    • Athena Electronic Health Records (EHR) experience
