Pharmacist-Care for You

Location:

Remote, patient home visits, and weekly team meeting in person

Terms:

Full-time salary/Monday-Friday-may occasionally receive phone calls on weekends from clinical staff

About the role:

We are looking for a pharmacist to provide home visits performing comprehensive medication review in identifying, resolving, and preventing medication- related problems to minimize hospitalizations and emergency room visits. Integrated with patient care team, the pharmacist will make recommendations, educate patient, caregiver, and evaluate focusing on medication use in the frail and elderly population living at home. The pharmacist will seek to establish opportunities for team education with goal of improving chronic disease outcomes. This position will report to Clinical Services Operations Manager

Responsibilities:

• Comprehensive medication reviews
• Telephone, Virtual, and in-person medication therapy management sessions.
• The pharmacist will provide the care team with information regarding dosage, interactions.
• Discuss potential effects of medication: including dietary and other considerations.
• Instruct patients and caregivers on devices for blood pressure and diabetes monitoring

Candidate requirements:

• Pharm D degree required; active license to practice pharmacy in New York State in good standing required.

• Three (3) years of pharmacy experience preferred.
• Ambulatory experience preferred
• Exceptional attention to detail
• Self-starter with ability to work proactively and take initiative
